Hintergrund
Der operative Ersatz des Hüft- oder Kniegelenks durch ein künstliches Gelenk ist die unter anderem am häufigsten durchgeführte Operation im Fachbereich der Orthopädie. Im Hinblick auf den erwarteten Anstieg an Patientenzahlen und die aktuellen Herausforderungen wie die COVID-19-Pandemie können digitale Gesundheits-applikationen zu einer besseren und kosteneffizienteren Versorgung beitragen. Während ihr Einsatz in der postoperativen Rehabilitation bereits ausführlich untersucht wurde, blieb eine Betrachtung der perioperativen Versorgung bisher außen vor. Ziel dieser Arbeit ist es, die Effekte und Auswirkungen von digitalen Gesundheits-applikationen auf die perioperative Versorgung von Patienten mit Hüft- oder Kniegelenksersatz darzustellen.
Methode
Ende November 2021 wurde eine systematische Literaturrecherche in den Datenbanken PubMed, MEDLINE und Embase via Ovid sowie der Web of Science Core Collection durchgeführt. Die erfassten Werke wurden auf ihre methodische Qualität hin untersucht und systematisch in eine Datentabelle extrahiert.
Ergebnisse
Von den 1302 identifizierten Studien wurden 19 aufgrund der Ein- und Ausschluss-kriterien in diese Arbeit inkludiert. Anhand der heterogenen Studienlage können positive Effekte digitaler Gesundheitsapplikationen auf die Inanspruchnahme des Gesundheitswesens, die postoperativen klinischen Ergebnisse, die PROMs sowie die Kosten angenommen werden. Bei den betrachteten Applikationen zeigte sich eine durchwegs hohe Benutzerfreundlichkeit und Akzeptanz bei den Endanwendern.
Schlussfolgerung
Digitale Gesundheitsapplikationen können zu einer kosteneffizienten Behandlung mit einer zum Teil früheren oder besseren Genesung beitragen. Hierbei zeigte sich insbesondere eine Überlegenheit bei Patienten mit Hüftgelenksersatz hinsichtlich möglicher Kosteneinsparungen und den klinischen Ergebnissen. Aufgrund der begrenzten methodischen Qualität und Studienlage bedarf es weiterer Unter-suchungen, um belastbare Aussagen treffen zu können.
Background
Surgical replacement of the hip or knee joint with an artificial joint is one of the most frequently performed operations in the field of orthopedics. In view of the expected increase in patient numbers and current challenges such as the COVID-19 pandemic, digital health applications can contribute to a better and more cost-effective care. While their use in postoperative rehabilitation has been extensively studied, a consideration of perioperative care has been left out. The aim of this paper was to present the effects and impact of digital health applications on the perioperative care of hip and knee replacement patients.
Methods
A systematic literature search was conducted at the end of November 2021 in the PubMed, MEDLINE, and Embase databases via Ovid and the Web of Science Core Collection. The collected works were examined for their methodological quality and systematically extracted into a data table.
Results
Of the 1,302 studies identified, 19 were included in this work according to the inclusion and exclusion criterias. Based on the heterogeneous set of studies, positive effects of digital health applications on healthcare utilization, postoperative clinical outcomes, PROMs, and costs can be assumed. The applications considered showed a consistently high level of usability and acceptance among end users.
Conclusion
Digital health applications can contribute to cost-efficient treatment with, in some cases, earlier or better recovery. In particular, digital health applications have been shown to be superior for hip replacement patients in terms of potential cost savings and clinical outcomes. Due to the limited methodological quality and study situation, further investigations are required in order to be able to make reliable statements.

Der operative Ersatz des Hüft- oder Kniegelenks (Arthroplastie) durch ein künstliches Gelenk (Endoprothese), in der Literatur oftmals unter dem Überbegriff der Total Joint Arthroplasty (TJA) zusammengefasst, ist die mit am häufigsten durchgeführte Operation (OP) im Fachbereich der Orthopädie. Während 2017 in den Vereinigten Staaten von Amerika (USA) bereits mehr als 960 000 Patienten mit Knie- sowie über 640 000 mit Hüftendoprothesen behandelt wurden (iData Research, 2018), wird bis zum Jahr 2025 mit einem Anstieg der Anzahl an Arthroplastiken der Hüfte um 75 % sowie des Knies um 110 % gerechnet (Singh et al., 2019). Neben dem demografisch bedingten Zuwachs an Patientenzahlen restringiert zudem die seit Ende 2019 andauernde Coronavirus Disease 2019 (COVID-19) Pandemie (World Health Organization, 2020) die orthopädischen Behandlungskapazitäten und führte so zu einem zeitweisen Rückgang der zumeist elektiven Eingriffe von bis zu minus 88 % (Kienzle et al., 2021). Hinzukommend erschwert der durch die COVID-19-Pandemie zum Teil eskalierte Mangel an medizinischen Fachpersonal (Blum et al., 2019) sowie der stetige Trend zu einer Reduktion der stationären Verweildauer im Sinne eines höheren Patientenumsatzes (Klauber et al., 2021) die Behandlung von orthopädischen Patienten. Eine Option sowohl dem Zuwachs an Patientenzahlen als auch einer Kostenexplosion im deutschen Gesundheitssystem unter Wahrung der medizinischen Versorgungsqualität zu begegnen, bieten digitale Gesundheitsapplikationen. Mögliche Anwendungsgebiete reichen hierbei vom Einsatz im privaten Bereich bis hin zur Nutzung im öffentlichen Gesundheitswesen. Von der Erfassung der persönlichen Schrittzahl (Lyman et al., 2020) über die Diagnose von Krankheiten durch Künstliche Intelligenz (KI) (Chung et al., 2018) bis hin zur Konsultation von Patienten mittels Telemedizin (Bini & Mahajan, 2017) scheinen modernen Gesundheitstechnologien keine Grenzen mehr gesetzt. Während die positiven Effekte von digitalen Gesundheitsapplikationen auf die postoperative Rehabilitation nach Hüft- oder Kniegelenksersatz bereits im Rahmen einer systematischen Literaturübersicht untersucht wurden, blieben die Auswirkungen auf die perioperative Versorgung bisher außen vor (McKeon et al., 2021). In Anbetracht dieses Hintergrundes und der beschriebenen Entwicklungen in der Orthopädie wurde die Forschungsfrage für die vorliegende Arbeit deshalb wie folgt formuliert:
Welche Effekte und Auswirkungen hat der Einsatz von digitalen Gesundheitsapplikationen auf die perioperative Versorgung von Patienten mit Hüft- oder Kniegelenksersatz?
Digitale Technologien und Anwendungen haben seit der Erfindung des ersten Smartphones 1992 eine unaufhaltsame Ausbreitung in alle gesellschaftlichen Bereiche und Industrien erfahren (Sager, 2012). So hat sich der Anteil an US-Amerikanern mit einem Smartphone innerhalb der letzten zehn Jahre von 35 % auf 85 % mehr als verdoppelt (Pew Research Center, 2021). Bei einem Gesamtwert des weltweiten Marktes für mobile Applikationen von über 154 Milliarden Dollar, beanspruchten davon Gesundheitsapplikationen bereits 2019 mehr als 10 % (Grand View Research, 2020).
Angesichts dieses Trends folgte auch 2019 in Deutschland durch die Verabschiedung des Digitale-Versorgung-Gesetzes (DVG) eine Aufnahme der digitalen Gesundheits-anwendungen (DiGA) in das Sozialgesetzbuch (SGB) V, womit der Weg für eine Kosten-übernahme durch die gesetzlichen Krankenkassen geebnet wurde (Deutscher Bundestag, 2019). Das Bundesinstitut für Arzneimittel und Medizinprodukte definiert eine DiGA als ein Medizinprodukt, dessen Hauptfunktion auf digitalen Technologien beruht, vom Leistungserbringer und Patienten gemeinsam genutzt werden kann und einen positiven Versorgungseffekt mit sich bringt (Bundesinstitut für Arzneimittel und Medizinprodukte, 2021). Damit eine digitale Gesundheitsapplikation den Status einer erstattungsfähigen DiGA im deutschen Gesundheitssystem erreichen kann, erfolgt eine Prüfung der genannten Anforderungen sowie unter anderem der Sicherheit und des Datenschutzes durch das BfArM im Rahmen eines Fasttrack-Verfahrens (vgl. Abbildung 1). Nach der erfolgreichen Begutachtung wird spätestens drei Monate nach Antragsstellung eine Entscheidung über die Aufnahme der Applikation in das DiGA-Verzeichnis getroffen. Sofern für die DiGA noch kein Nachweis eines positiven Versorgungseffektes im Rahmen einer wissenschaftlichen Studie vorliegt, muss dieser innerhalb von 12 Monaten nach Aufnahme erbracht werden – andernfalls erfolgt die Streichung aus dem Verzeichnis. Im Anschluss an die Aufnahme in das Verzeichnis findet eine Verhandlung zwischen dem Hersteller und dem Spitzenverband Bund der Krankenkassen über die Höhe der Vergütung statt. Zum Stand des 04. Dezember 2021 befanden sich 24 erstattungsfähige digitale Gesundheitsanwendungen im DiGA-Verzeichnis (ebd.).

Aufgrund der nicht ausreichenden Studienlage zu den DiGAs im untersuchten Indikationsgebiet sowie den länderspezifischen Anforderungen, wurde im Rahmen dieser Arbeit keine Eingrenzung hinsichtlich des Vorliegens eines erstattungsfähigen Medizinproduktes vorgenommen. Viel mehr lag der Fokus auf der Untersuchung von Applikationen, welche durch die Nutzung von digitalen Technologien die medizinische Versorgung verbessern wollen. In Anbetracht dessen wurde nachfolgend keine Abgrenzung zwischen digitalen Applikationen und erstattungsfähigen DiGAs vorgenommen.
Der operative Gelenksersatz des Knies oder der Hüfte findet primär Anwendung nach Versagen von konservativen Therapieformen, wie der physikalischen oder medikamentösen Therapie, beim chronisch-progredienten Gelenksverschleiß (Arthrose). Weitere Indikationen stellen beispielsweise Entzündungen, Tumore oder auch gelenksnahe Frakturen dar (Rahm & Zingg, 2018). Nach Einzelfallabwägung kann dabei neben dem kompletten Gelenksersatz, der Total Knee Arthroplasty (TKA) oder Total Hip Arthroplasty (THA), auch ein nur partieller, gelenkserhaltender Ersatz mittels einer Hemiprothese vorgenommen werden.
Im Rahmen der OP-Vorbereitung findet präoperativ neben der Bildgebung und anästhetischen Vorbereitung zudem eine Antibiotika- sowie Thromboseprohyplaxe zur Vorbeugung von Infektionen und Embolien statt. Weitere mögliche Komplikationen der operativen Behandlung stellen ferner Blutverluste, eine Dislokation oder auch ein Versagen der Prothese dar. Außer den Komplikationen können zudem Begleiterkrankungen (Komorbiditäten) den Heilungsprozess verzögern und somit die stationäre Verweildauer verlängern oder eine Revisionsoperation nötig werden lassen. Im Anschluss an den Eingriff liegt deshalb ein Hauptaugenmerk auf einer adäquaten Schmerztherapie, der Wiederherstellung der Funktionsfähigkeit durch Therapieeinheiten sowie der rechtzeitigen Erkennung möglicher Komplikationen. Mit einer annähernden Beschwerdefreiheit ist in Abhängigkeit des Allgemeinzustandes nach sechs bis zwölf Wochen postoperativ zu rechnen (Witjes et al., 2016).
Zur Ergänzung der regulären Nachsorgetermine sowie als Möglichkeit der Erfassung und Quantifizierung dieser zum Teil nur subjektiv erkennbaren Therapieergebnisse und Komplikationen, können Patient-Reported Outcome Measures (PROMs) genutzt werden. Weldring und Smith (2013) beschreiben Zweck und Zielsetzung eines Patient-Reported Outcome (PRO) als „directly reported by the patient without interpretation of the patient’s response by a clinician or anyone else and pertains to the patient’s health, quality of life, or functional status associated with health care or treatment.” PROMs bieten neben der Erhebung von einzelnen Größen wie dem Schmerz oder der Stimmung auch die Möglichkeit der Kombination mehrerer Dimensionen in einem übergeordneten Score. So messen beispielsweise der Oxford Knee Score (OKS), der Oxford Hip Score (OHS) und der Harris Hip Score (HHS) sowohl mögliche Schmerzen als auch die Funktionalität des Gelenks (Johnston et al., 1990; Murray et al., 2007). Eine etwas umfangreichere Abbildung des Gesundheitszustandes ermöglichen dagegen der Knee Injury and Osteoarthritis Outcome Score (KOOS) (Roos & Lohmander, 2003), der Oswestry Very Short Form (VSF) (Blocker et al., 2017) sowie der Hip Disability and Osteoarthritis Outcome Score (HOOS) (Nilsdotter et al., 2003). Diese erfassen neben den Schmerzen und der Funktionalität auch weitere Symptome sowie die Lebensqualität (Quality of Life). Die Quality of Life (QOL) im Sinne des subjektiven Wohlbefindens lässt sich zudem auch mittels des European Quality of Life 5 Dimensions 3 Level Version (EQ-5D-3L) Scores erfassen (EuroQol Group, 1990). Ein weiterer in dieser Arbeit erwähnter PROM ist der Self-Efficacy for Managing Chronic Disease (SEMCD) Score, welcher die erlebte Selbstwirksamkeit des Individuums im Umgang mit den krankheitsbezogenen Symptomen misst (Ritter & Lorig, 2014). Einen etwas breiteren Ansatzpunkt bieten PROMs zur Erfassung der Patientenzufriedenheit wie der Consumer Assessment of Healthcare Providers and Systems (CAHPS) (Giordano et al., 2010) oder der Picker Patient Experience (PPE-15) (Jenkinson et al., 2002) Score, welche die gesamte Versorgungsqualität zu erfassen versuchen.
Bei der Beantwortung von Forschungsfragen sowie im Zuge der hierfür notwendigen Informationsgewinnung sehen sich mögliche Interessensgruppen im Rahmen einer einfachen Literaturrecherche mit einer Vielzahl von Problematiken konfrontiert:
Während sich 2010 das medizinische Wissen noch alle 3,5 Jahre verdoppelte, wurde für das Jahr 2020 bereits eine Verdoppelung auf alle 73 Tage geschätzt (Densen, 2011). Doch bereits 2002 hätten Fachärzte 627,5 Stunden jeden Monat benötigt, um ihr Wissen auf dem neuesten Stand der Forschung zu halten (Alper et al., 2004). Neben der Informationsflut stellt dabei auch die in der Literatur vorherrschende Heterogenität sowie die für den Laien oftmals nur schwer nachvollziehbare Qualität der einzelnen Arbeiten ein weiteres Hindernis in der Bewertung wissenschaftlicher Studien dar. Auch angesichts der zum Teil widersprüchlichen Ergebnisse einzelner Studien zu ähnlichen oder sogar gleichen Forschungsfragen ist von einer unstrukturierten Betrachtung der verfügbaren Evidenz nur abzuraten. Denn gerade in Bereichen mit Bezug zur Patientenversorgung müssen mögliche Stakeholder, vom Patienten über den behandelnden Arzt bis hin zu den politischen Entscheidungsträgern, belastbare Aussagen und Entscheidungen treffen können, um so möglicherweise letale Ausgänge abwenden zu können. Eine Grundlage hierfür kann nur eine empirisch beleg- und überprüfbare Datenbasis bieten.
Eine Abhilfe in der Bewältigung dieser möglicherweise bestehenden Schwierigkeiten in der Evidenzsynthese kann eine systematische Literaturübersicht bieten. Higgins et al. (2019) beschreiben die Zielsetzung dieser Forschungsmethode wie folgt:
„A systematic review attempts to collate all the empirical evidence that fits pre-specified eligibility criteria in order to answer a specific research question. It uses explicit, systematic methods that are selected with a view to minimizing bias, thus providing more reliable findings from which conclusions can be drawn and decisions made.“
Eine systematische Literaturübersicht verfolgt demnach mehrere Ansprüche:
Ausgehend von einer a priori bestimmten Forschungsfrage wird die gesamte verfügbare Literatur anhand von definierten Kriterien systematisch durchsucht, um so diese möglichst gänzlich zu beantworten. Gleichwohl kann im Rahmen dessen auch zu der Erkenntnis gelangt werden, dass die verfügbare Evidenz eine Beantwortung nicht erlaubt und somit der zukünftigen Forschung ein wichtiger Hinweis für eine vertiefte Untersuchung gegeben werden. Des Weiteren können anhand von wissenschaftlich anerkannten Richtlinien, wie beispielsweise des Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 Statement (M. J. Page et al., 2021), mögliche Schwachstellen und Mängel der Studien in der Zusammentragung und Bewertung der Evidenz berücksichtigt werden.
Dieses Instrument der Informationssynthese bietet daher aufgrund seines kompendiösen Charakters eine strukturierte und nachvollziehbare Möglichkeit der Evidenzsynthese und ermöglicht im Zuge derer den Entscheidungsträgern die Ableitung von belastbaren Aussagen und Entscheidungen.
Ein zentrales Kennzeichen von systematischen Literaturübersichten ist demnach die Verfolgung einer strukturierten methodischen Vorgehensweise bei der Beantwortung der Fragestellung. Vorliegende Studie orientierte sich hinsichtlich des Aufbaus und der Struktur am PRISMA 2020 Statement. Dieses stellt eine durch eine Autorengruppe überarbeitete Version des in der Literatur über 60 000-mal verwendeten PRISMA 2009 Statement dar, welches mittels einer evidenzbasierten Checkliste von 27 Punkten die Erstellung systematischer Literaturübersichten standardisieren und verbessern möchte (ebd.). Im Falle von Unklarheiten wurde auf das seit 1994 von der renommierten Organisation Cochrane herausgegebene Cochrane Handbook for Systematic Reviews of Interventions zurückgegriffen. Cochrane hat sich als weltweiter Zusammenschluss von Stakeholdern der Gesundheitsbereiche im Rahmen ihrer Arbeit primär der Verbesserung und Verbreitung der evidenzbasierten Entscheidungsfindung ver-schrieben (Higgins et al., 2019).
Zur Konzeption einer geeigneten Forschungsfrage wurde auf das erstmals von Richardson et al. (1995) vorgeschlagene PICO-Schema zurückgegriffen. Demnach sollte eine Forschungsfrage vier grundlegende Bestandteile beinhalten: „1) the patient or problem being addressed; 2) the intervention or exposure being considered; 3) the comparison intervention or exposure, when relevant; 4) the clinical outcomes of interest.“ (ebd.) Angelehnt an dieses Konzept wurde die Forschungsfrage für die vorliegende Arbeit wie folgt gestellt: Welche Effekte und Auswirkungen hat der Einsatz von digitalen Gesundheitsapplikationen auf die perioperative Versorgung von Patienten mit Hüft- oder Kniegelenksersatz?
Ausgehend von dieser Fragestellung sowie unter Berücksichtigung der bereits verfügbaren Evidenz wurden folgende Ein- und Ausschlusskriterien festgelegt:
Eingeschlossen wurden alle innerhalb der letzten zehn Jahre publizierte Studien, welche die Auswirkungen des Einsatzes von webbasierten oder mobilen digitalen Gesundheitsapplikationen auf die prä- oder postoperative Behandlung von Patienten mit Hüft- oder Kniearthroplastik untersuchten. Inkludiert wurden des Weiteren Arbeiten, welche die Nutzung von mobilen Textnachrichten als Untersuchungs-gegenstand zu eigen hatten. Es wurde keine Eingrenzung hinsichtlich einer Erst- oder Revisionsoperation, der Operationsmethode bzw. Prothesenart, des Studienlandes oder der Demografie vorgenommen, mit Ausnahme des Alters von mindestens 18 Jahren. Ausgeschlossen wurden hingegen alle Studien, die nicht in deutscher oder englischer Sprache verfasst worden waren oder deren Volltext nicht zugänglich war. Außerdem wurden angesichts der bereits existierenden systematischen Literaturübersichten zur Verwendung von Smartwatches und externen Sensoren (Shayan Bahadori et al., 2020) und zum Einsatz digitaler Technologien in der postoperativen Rehabilitation (McKeon et al., 2021) alle Studien mit diesem Fokus ausgeschlossen. Dabei wurden jedoch keine Arbeiten exkludiert, welche die Verwendung von bereits in gängigen Smartphones verbauten Sensoren untersuchten. Ebenfalls wurden rein qualitative oder narrative Studien sowie Arbeiten mit Fokus der Verwendung digitaler Applikationen in der intraoperativen Versorgung, zur Bewegungsumfangs- oder Ganganalyse, zur Validierung von digitalen Mess-instrumenten sowie der reinen Untersuchung von Videoanrufen im Rahmen einer virtuellen Verlaufskontrolle oder Nachbehandlung ausgeschlossen.
Da keine wissenschaftliche Datenbank existiert, die einen Anspruch auf Vollständigkeit ihrer Suchergebnisse erhebt, empfiehlt sich eine Literatursuche in mehreren biomedizinischen Datenbanken, um systematischen Fehlern in der Recherche vorzubeugen. Die Literaturrecherche erfolgte deshalb Ende November 2021 durch den Autor in den wissenschaftlichen Datenbanken PubMed, MEDLINE und Embase via Ovid sowie der Web of Science (WoS) Core Collection. Im ersten Schritt wurde explorativ nach potenziellen Titeln gesucht, um die relevanten Schlüsselwörter und Begriffe für die systematische Literatursuche zu identifizieren. Anhand dieser wurden die für die einzelnen Datenbanken in Tabelle 1 dargestellten Suchstrings entwickelt. Die letzte Datenbankabfrage erfolgte am 28. November 2021. Bei der Literatursuche wurden keine deutschen Suchstrings verwendet, da angenommen wurde, dass auch deutsche Arbeiten ein englisches Abstrakt enthalten. Möglicherweise wurden dadurch nicht alle relevanten Artikel von der Suche erfasst. Zur Ergänzung der Datenbanksuche erfolgte abschließend eine Handsuche nach bisher nicht erfassten Werken über die Literaturverzeichnisse der bereits inkludierten Arbeiten.

Um eine Vergleichbarkeit der eingeschlossenen Artikel im Rahmen der Evidenzsynthese zu ermöglichen, wurden durch den Autor wesentliche Merkmale der eingeschlossenen Studien in einer Datentabelle narrativ zusammengefasst. Dabei handelte es sich um die Kategorien Autor, Jahr, Land, Studiendesign und -größe, zentrale Charakteristika der Interventions- und sofern vorhanden Kontrollgruppe, dem Studienziel, die verwendeten Methoden, den Erhebungszeitraum sowie die wesentlichen Ergebnisse. Im Vorfeld der Extraktion erfolgte dabei eine Testung und Spezifizierung der Datenextraktion mittels der angesprochenen Kategorien über nicht eingeschlossene Studien. Der Übersichtlichkeit wegen erfolgte eine Kategorisierung der berichteten Ergebnisse in die Rubriken Befragung, Datenanalyse und PROMs. Für statistisch signifikante Ergebnisse bei einem Signifikanzniveau von kleiner oder gleich 5 % wurde zudem der p-Wert mit angegeben.

Ein wesentlicher Bestandteil der Methodik einer systematischen Literaturübersicht stellt eine strukturierte und nachvollziehbare Qualitätsbewertung der eingeschlossenen Studien dar. Ziel hiervon ist es, dem Leser durch die Darstellung möglicher systematischer Fehler in den eingeschlossenen Arbeiten eine gewichtete Interpretation der einzelnen Ergebnisse zu erlauben (Matthew J. Page et al., 2021). Dabei ergänzt und vertieft diese Methodik die Erfassung der Validität von wissenschaftlichen Studien durch die häufig verwendete Einordnung in eine Evidenzhierarchie anhand des Studientypus (vgl. Tabelle 2).

Zur Qualitätsbeurteilung wurden das von Cochrane vorgeschlagenen Risk-of-Bias (RoB) 2 Tool für randomisierte kontrollierte Studien (Sterne et al., 2019) sowie das Risk Of Bias In Non-Randomized Studies - of Interventions (ROBINS-I)Tool für nicht randomisierte Studientypen verwendet (Sterne et al., 2016). Im Anschluss erfolgte eine Visualisierung der Ergebnisse der Qualitätsbewertung mit Hilfe des Risk-of-bias VISualization (robvis) Tools (McGuinness & Higgins, 2020).
Das RoB-2 Tool analysiert das Risiko des Vorliegens systematischer Fehler bei randomisierten Studien anhand von insgesamt 28 Fragen zu den Bereichen Rando-misierung (D1), Abweichungen von der geplanten Intervention (D2), fehlende Daten (D3), der Art der Messung (D4) sowie der Auswahl der Ergebnisse (D5). Über einen vorgegebenen Algorithmus erfolgt anschließend eine abschließende Bewertung des Artikels in „low risk“, „some concerns“ und „high risk“ hinsichtlich des Vorliegens von systematischen Fehlern. Zu beachten ist, dass bereits das einmalige Vorliegen von „some concerns“ oder „high risk“ in einem der fünf Bereiche die Gesamtbewertung bestimmt (Sterne et al., 2019).
Das ROBINS-I Tool hingegen findet Anwendung bei allen nicht randomisierten Studien. Neben der Risikoeinschätzung für das Vorliegen von systematischen Fehlern verfolgt es zudem den Anspruch, eine Gleichwertigkeit der untersuchten nicht randomisierten Studien zu randomisierten kontrollierten Studien festzustellen. Aus diesem Grund erhalten Studien einer Evidenzstufe von vier oder niedriger relativ leicht eine mangel-hafte Bewertung, auch wenn deren Autoren diesen Anspruch auf methodische Äquivalenz vielleicht nie verfolgt hatten. Die Risikoeinschätzung erfolgt im Rahmen dieses Tools mittels insgesamt 41 Fragen zu den Bereichen Störfaktoren (D1), Probandenauswahl (D2), Klassifizierung der Intervention (D3), Abweichungen von der geplanten Intervention (D4), fehlende Daten (D5), der Art der Messung (D6) und Auswahl der Ergebnisse (D7). Die abschließende Risikoeinschätzung erfolgt nach „low risk“, „moderate risk“, „serious risk“ und „critical risk“. Gleichsam wie beim RoB-2 Tool bestimmt das einmalige Vorliegen von „moderate risk“, „serious risk“ oder „critical risk“ in einem der sieben Bereiche bereits die Gesamtbewertung (Sterne et al., 2016).
Nachfolgend werden die Ergebnisse der systematischen Literaturrecherche mit der zugrundeliegenden Selektionsmethodik beschrieben. Eine schematische Darstellung dieses Prozesses findet sich in Abbildung 1. Mittels der unter Abs. 3.2.2 beschriebenen Suchstrategie konnten 1 302 Treffer in den vier beschriebenen Datenbanken erfasst werden. Hiervon fielen 315 auf PubMed, 186 auf MEDLINE via Ovid, 325 auf Embase sowie 476 auf die Web of Science Core Collection. Unter Verwendung der Literaturverarbeitungssoftware EndNote 20.1 wurden diese anschließend um 654 Duplikate bereinigt und dadurch auf 655 Treffer reduziert. Im Anschluss erfolgte eine Sichtung der verbliebenen Treffer durch den Autor im Rahmen eines iterativen Prozesses anhand der festgelegten Ein- und Ausschlusskriterien. Durch ein einfaches Titel und Abstrakt Screening konnten dabei 485 Artikel im Hinblick auf ihre fehlende Relevanz für die Beantwortung der Fragestellung ausgeschlossen werden. Von den übrigen 170 Treffern wurden weitere zehn aufgrund eines nicht verfügbaren Volltextes exkludiert. Durch die Prüfung von 160 Volltexten auf ihre Eignung konnten weitere 141 Treffer ausgeschlossen werden. Hiervon beschäftigten sich 77 Artikel mit der intraoperativen Anwendung von digitalen Applikationen sowie 13 mit der Untersuchung der Validität einer softwaregestützten Gang- oder Bewegungsumfangsanalyse. Weitere 14 Artikel untersuchten den Einsatz von digitalen Gesundheitsapplikationen im Rahmen der postoperativen physikalischen

Rehabilitation sowie 19 den Nutzen eines Einsatzes von Smartwatches oder Sensoren in der unmittelbaren Patientenversorgung. Ferner wurden 18 Publikationen aufgrund verschiedener Gründe, wie beispielsweise des Studientypus oder der Sprache, exkludiert und dabei in der Abbildung 1 unter „Sonstiges“ subsumiert. Schlussendlich konnten 19 Artikel in diese Arbeit einge-schlossen werden, wovon sieben durch eine Handsuche der Literaturverzeichnisse der aus der Datenbankrecherche inkludierten Werke identifiziert wurden.
Die älteste eingeschlossene Studie wurde am 16. Mai 2017 publiziert (Blocker et al., 2017), die aktuellste am 24. April 2021 (Knapp et al., 2021). Die Studiengröße variierte bei den eingeschlossenen Arbeiten von 17 (Blocker et al., 2017) bis hin zu 6 426 Probanden (Plate et al., 2019). Von den 19 eingeschlossenen Studien untersuchten elf (Campbell et al., 2019; Day et al., 2018; Gwam et al., 2019; Knapp et al., 2021; Lu et al., 2018; Lyman et al., 2020; Plate et al., 2019; Rosner et al., 2018; Visperas et al., 2021; Yahanda et al., 2019; Zhang et al., 2021) mögliche Effekte anhand einer Population aus den USA, drei (Pronk et al., 2020; Scheper et al., 2019; Timmers et al., 2019) aus den Niederlanden (NL), je zwei aus dem Vereinigten Königreich (UK) (Blocker et al., 2017; Jayakumar et al., 2017) und China (CHN) (Zhang et al., 2021; Zheng et al., 2019) sowie eine (Saunders et al., 2021) aus Australien (AUS).
