This critical literature review aims to investigate the role of physical exercise in the prevention and management of depression among inpatients, evaluating its efficacy as a complementary therapeutic intervention.
Psychosocial and Biological Mechanisms Around Exercise
The second theme was developed using evidence from the studies included in this review about the psychosocial and biological mechanisms around exercise and their impacts on depressive symptoms among the inpatients. Evidence from the studies by Godino et al. (2014) and Smith et al. (2019) shows that exercise activates the peroxisome proliferator-activated receptor γ activating factor 1α (PGC-1α) gene present in the skeletal muscle which leads to the modulation of the Kynurenine (KYN) metabolism and improves ability of an individual to be resilient to stress-induced depression. Similar arguments are presented in the study by Dickinson et al. (2023) which reports that the mechanism by which the expression of exercise-induced skeletal muscle PGC-1α leads to the improvement of depressive behaviours is primarily through the regulation of KYN metabolism, expression of the central brain-derived neurotrophic factor (BDNF) and peripheral inflammatory factors, hence having indirect antidepressant effects.
With reference to the arguments by Godino et al. (2014), endurance exercise among the inpatients facilitate the creation of a balance in the skeletal KYN metabolism via the PGC-1α pathway, leading to the alleviation of the neurotoxic effects of KYN, and exertion of the antidepressant effects. Consistent evidence has been reported in the study by Smith et al. (2019) which establishes that aerobic exercise has positive impacts on the skeletal muscle PGC-1α and KAT1-4 expression among depressed individuals. Therefore, it is justifiable to note that exercise leads to the activation of skeletal muscle PGC-1α which plays an important role in the regulation of peripheral KYN metabolism, reduction in the peripheral inflammatory response as well as secretion of neuroprotective factors, hence exerting antidepressant effects among the inpatients.
