Radiojodtherapie benigner und maligner Schilddrüsenerkrankungen
Die Radiojodtherapie (RJT) ist seit über 60 Jahren ein etabliertes Verfahren zur Behandlung sowohl von malignen als auch benignen Schilddrüsenerkrankungen. Das differenzierte Schilddrüsenkarzinom [„differentiated thyroid carcinoma“ (DTC)] umfasst die papillären und follikulären Karzinome, welche mit etwa 90% den Großteil der malignen Schilddrüsenneoplasien ausmachen. Für die selteneren anaplastischen und medullären Schilddrüsenkarzinome besteht wegen fehlender Jodanreicherung keine Indikation zur RJT. Zu den benignen Formen zählen die Immunhyperthyreose, die funktionelle Autonomie und die Struma [1].
Die Wirksamkeit der RJT beruht auf der biologischen Fähigkeit der Schilddrüsen- und der differenzierten Schilddrüsenkarzinomzellen, Jodid zu speichern. Für die Radionuklidtherapie hat sich das Jod-131 (131I) bewährt: 131I mit einer Halbwertszeit von 8,0 Tagen emittiert sowohl β_- als auch y-Strahlung. Die therapeutische Wirkung wird durch die β~-Strahlung erzielt. Ihre kurze mittlere Reichweite im Gewebe von 0,8 mm und die hohe selektive Anreicherung haben zur Folge, dass fast nur das jodspeichernde Gewebe bestrahlt wird. Schäden des umgebenden Gewebes können dadurch größtenteils vermieden werden. Die durchdringende y-Strahlung mit einer Energie von 364 keV kann an der Körperoberfläche mit Gammakameras oder Sonden registriert werden, so dass die Jodspeicherung zum Beispiel szintigraphisch visualisiert werden kann.
Die Therapiekonzepte sind für die malignen und benignen Schilddrüsenerkrankungen verschieden:
- Die RJT des DTC [2] wird adjuvant nach primär chirurgischer Thyreoidektomie zur Ablation von Restschildrüsengewebe und zur Therapie von Tumorresten oder Tumorrezidiven sowie von Lymphknoten- und/oder Fernmetastasen eingesetzt. Bei Patienten vor Ersttherapie erfolgt in unserer Klinik ein Radiojodtest [3] zur Bestimmung des Radiojoduptakes der Restschilddrüse nach 24 h; bei einem Uptake >20% muss eine Reoperation in Erwägung gezogen werden. Trotz der insgesamt guten Prognose [4] beträgt die kumulative Rezidivrate bis zu 30% [5]. Auch nach Jahren können noch Rezidive auftreten: lokale und regionäre Rezidive bei 5 bis 20% und Fernmetastasen bei < 10% der Patienten [6]. Häufige Nebenwirkungen bei hochdosierter, mehrfacher 131I-Applikation sind Spätkomplikationen wie Mundtrockenheit (Xerostomie) als Folge der radiogenen Speicheldrüsenentzündung [7,8,9,10]. Knochenmarkstoxizität sowie Leukämie und Sekundärkarzinome sind sehr selten und treten eher bei hohen kumulativen Aktivitäten auf [2,11,12].
Die nationalen und internationalen Leitlinien [2] zur Durchführung der RJT des DTC sehen im Regelfall therapeutische Standardaktivitäten vor (2 bis 5 GBq 131I bei Ablation, 4 bis 11 GBq 131I bei Rezidiven, Lymphknoten- und Fernmetastasen). Auf eine individuelle prätherapeutische Dosimetrie, d. h. eine Abschätzung der individuell erforderlichen therapeutischen 131I-Aktivitäten zur Beseitigung von Tumorresten oder Tumorrezidiven sowie von Metastasen vor der RJT, wird in der Regel verzichtet, da die auf Gammakameratechnik basierende Dosimetrie zeitaufwendig und mit einem - im Messprinzip begründeten - hohen Fehler behaftet ist [4,13,14,15].
- Im Gegensatz dazu ist bei der RJT benigner Schilddrüsenerkrankungen [16] eine individuelle prätherapeutische Dosimetrie in Deutschland leitliniengemäß in aller Regel gefordert [17,18]. Dazu wird vor der Therapie dem Patienten eine Testaktivität verabreicht und die Biokinetik (Radiojoduptake und effektive Halbwertszeit) mit einer Messsonde bestimmt [3]. Ziel ist die prätherapeutische Bestimmung der Therapieaktivitäten, um die gewünschte absorbierte Dosis im Zielvolumen zu erreichen. In unserer Klinik variieren die therapeutischen Aktivitäten zwischen 0,2 bis 1,6 GBq 131I. Die individuelle Dosimetrie ist mit einem guten Therapieerfolg verbunden und Spätfolgen sind aufgrund der niedrigeren Aktivitäten unwahrscheinlich [19].
Auch wenn die RJT seit Jahren etabliert ist und gute Therapieerfolge vorzuweisen hat, ist eine Optimierung hinsichtlich der Therapieplanung und Minimierung von Nebenwirkungen anzustreben. Außerdem gibt es Bestrebungen, neuere Methoden wie zum Beispiel die Positro- nen-Emissions-Tomographie (PET) mit Jod-124 (124I) in die Therapieplanung des DTC [2,20] zu integrieren, weil PET derzeit das genaueste Verfahren zur In-vivo-Bestimmung von lokalen Aktivitätskonzentrationen im Gewebe ist.
Das Ziel der dieser kumulativen Habilitation zugrundeliegenden Arbeiten war die Etablierung und Evaluierung neuer Konzepte zur Optimierung der RJT. Die Gliederung der vorliegenden Schrift zeigt Abb. 1: Schwerpunkt ist die individuelle prätherapeutische 124I-PET-Dosimetrie beim DTC. Ein weiterer Punkt ist die Erhöhung der Genauigkeit des Radiojoduptakes beim Radiojodtest. „Optimierung“ bedeutet aber auch, Nebenwirkungen zu untersuchen und zu minimieren. Am Beispiel der Kopfspeicheldrüsen wurden die absorbierten Dosen bei der RJT des DTC ermittelt.

Individuelle prätherapeutische 124I-PET-Dosimetrie beim DTC
Bedeutung der 124I-Positronen-Emissions-Tomographie. Die nationalen und internationalen Leitlinien zur Durchführung der RJT des DTC basieren hauptsächlich auf retrospektiven Studien und Konsensusempfehlungen von internationalen Expertengruppen [2,16,21,22]. Dennoch variieren für die gleichen Patienten die Therapiedurchführung und -konzepte nach Land, Region und Institutionen zum Teil sehr stark.
In diesem Kontext ist daher ein diagnostisches bildgebendes Verfahren notwendig, das in der Lage ist, den Radiojoduptake in der Läsion (Tumorrest oder Tumorrezidiv, Metastase) genau und zuverlässig zu bestimmen. Ziel ist es, die RJT des DTC bei gleichzeitiger Minimierung der Strahlenexposition des Restkörpers optimal zu planen, eine individuelle Risiko-NutzenAnalyse durchzuführen und neue Interventionen objektiv zu evaluieren, die den Radiojoduptake beeinflussen (z.B. Verwendung von Vitamin A zwecks Redifferenzierung) [20].
In der Vergangenheit wurde Jod-123 (123I) und 131I zur individuellen prätherapeutischen Dosimetrie in Verbindung mit Gammakameras verwendet [23]. Die Verwendung dieser Radiojodisotope ist problematisch: Zum einen ist die Halbwertszeit von 123I (13,2 h) zu klein, um die Kinetik der Läsion ausreichend zu erfassen, und zum anderen müssen geringe prätherapeutische 131I-Aktivitätsmengen verwendet werden, um den Stunning-Effekt, d. h. die Verminderung des Radiojoduptakes bei der Therapie durch prätherapeutische Behandlung [24,25,26], nicht auszulösen. Die Verwendung von geringen 131I-Aktivitätsmengen hat wiederum den Nachteil, dass die Sensitivität zum Nachweis von jodspeicherndem Gewebe erniedrigt wird [2,26]. Insbesondere die Lungenmetastasen sind gelegentlich erst nach höheren 131I-Aktivitäten zu erkennen [27]. Darüber hinaus ist die auf Gammakamera-basierende prätherapeutische Dosimetrie zeitaufwendig und mit einem - im Messprinzip begründeten - hohen Fehler behaftet [4,13,14,15]:
In den letzten Jahren haben technische und radiochemische Innovationen neue Möglichkeiten auf diesem Gebiet eröffnet:
- Die PET ist das derzeit genaueste Verfahren zur In-vivo-Bestimmung von lokalen Aktivitätskonzentrationen im Gewebe und liefert, verglichen mit anderen bildgebenden Verfahren in der Nuklearmedizin, hochauflösende Schnittbilder der dreidimensionalen Aktivitätsverteilung. Die PET/Computertomographie(CT) ermöglicht außerdem eine exakte intrinsische Koregistrierung von funktionellen und morphologischen Informationen.
- Zur prätherapeutischen Dosimetrie ist das diagnostische Radionuklid der Wahl 124I [23,28,29,30,31,32,33,34,35]: 124I hat eine Halbwertszeit von 4,2 Tagen und ist daher im Gegensatz zu 123I sehr gut geeignet zur Erfassung der Läsionskinetik. Die applizierten Aktivitätsmengen betragen lediglich 20 bis 40 MBq [34]; bei diesen geringen Aktivitätsmengen sind Stunning-Effekte nicht zu erwarten und trotzdem ist im Gegensatz zu 131I die Sensitivität zum Nachweis von jodspeichernden Läsionen hoch [36,37,38].
- Die Arbeitsgruppe Radiochemie der Klinik für Nuklearmedizin in Essen kann 124I effizient und in hoher radiochemischer Reinheit herstellen [39,40].
Die Autoren [23] sind davon überzeugt, dass 124I-PET(/CT) eine große Rolle in der prätherapeutischen Dosimetrie und Radiojoddiagnostik des Schilddrüsenkarzinoms spielen wird und dass aufgrund des Potentials der genaueren Quantifizierung 124I-PET(/CT) grundsätzlich der Gammakamera mit 123I bzw. 131I überlegen ist [20,23,34,35,36,37,38]. Aufgrund des komplexen Zerfallsschemas von 124I ist allerdings eine Beeinträchtigung der Quantifizierung zu erwarten und muss gegebenenfalls korrigiert werden.
Quantitative Bildgebung von 124I mit PET. Das vereinfachte Zerfallschema (nur β+- und y- Übergänge) von 124I ist in Abb. 2 (links) wiedergegeben. Die Energien der emittierten
Positronen und der abgestrahlten y-Quanten sind in Abb. 2 (rechts) tabelliert. Der Positronenanteil (Anzahl der emittierten Positronen pro Zerfall) beträgt 22,8% und ist deutlich niedriger als das Standard-Radionuklid Fluor-18 (18F) mit einem Positronenanteil von 96,7% [41].

Die PET-Systeme verwenden Energiediskriminatoren. Die Standardwerte für die untere und obere Schwelle der Energiediskriminierung zur Erfassung der charakteristischen Annihilationsstrahlung (diametrale Aussendung von zwei Annihilationsquanten von je 511 keV Energie) sind oft 350 und 650 keV. Im Gegensatz zum Standard-Radionuklid 18F emittiert 124I eine Reihe von y-Quanten, die von den PET-Detektoren zusätzlich registriert werden und dadurch die PET-Quantifizierung beeinträchtigen.
Im Detail sind es die y-Quanten mit Energien von 603 keV und 723 keV (s. Abb. 2). Diese zusätzlichen y-Quanten werden entweder direkt oder indirekt nach Compton-Streuung erfasst. Dies hat zur Folge, dass in erster Linie die Rate der zufälligen Koinzidenzen und die Totzeit erhöht wird [42]. Noch wichtiger ist die Tatsache, dass etwa die Hälfte des Positronenzerfalls (52%) mit einer prompten (gleichzeitigen) Emission von 603-keV y-Quanten assoziiert ist. Mit anderen Worten handelt es sich dabei um eine Kaskade von zwei Quanten, einem Annihilationsquant und einem nachfolgenden, einzelnen y-Quant.
