This thesis aims to explore an integrated management approach for rosacea, combining medical therapies with specialized cosmetic strategies to alleviate symptoms and improve the patient's quality of life.
3.1 CHARACTERISTICS SENSITIVE SKIN
Sensitive skin is a condition of subjective skin hyper-reactivity induced by environmental factors. Individuals who experience this condition report exaggerated reactions when their skin comes into contact with cosmetics, soaps, and sunscreens, and often report worsening after exposure to dry, cold climates (E. Berardesca, 2013). The diagnosis, pathophysiology, epidemiology and treatment are still debated. The aetiology of sensitive skin is most likely due to neurogenic inflammation after enhanced activation of sensory proteins in keratinocytes and nerve endings (Misery, 2013).
The prevalence in Europe is relatively high, with approximately 50-60% of the population affected by sensitive skin, with a higher prevalence in women than in men (Farage, 2021). Furthermore, it has been found that its prevalence increases during the summer, it has been suggested that UV exposure may play a role in the appearance of sensitive skin (Misery, 2013), mainly due to immunosuppressive action following exposure to the sun. In support of this, light-skinned individuals prone to sunburn reported greater discomfort than those with darker skin, while other studies could not confirm an association with skin phototype (K. Loser, 2016).
Atopic dermatitis and other conditions, such as acne, rosacea, seborrheic dermatitis, contact dermatitis and psoriasis, promote the manifestation of a compromised skin barrier and evident inflammation (E. Berardesca, 2013). In this regard, there are subtypes of sensitive skin, among the most common: Allergic subtype, prone to redness, itching and scaling following exposure to various allergens or irritants. Previously described as cosmetic status or cosmetic intolerance syndrome. Patients with this skin subtype often experience contact urticaria, hives and an inflammatory response to fragrances and preservatives. This subtype is thought to be due to an increased immune response and impaired skin barrier. (Hong JY, 2020).
