Diese Arbeit entstand zur Erlangung des akademischen Grades eines Master of Science im Studiengang Therapiewissenschaften an der Hochschule Fresenius in Idstein. Die Patienten der MoVo-LISA-lnterventionsstudie wurden in den kardiologischen Rehabilitationskliniken „Lazariterhof“ und „Klinik Baden“ des Park-Klinikums in Bad Krozingen bei Freiburg rekrutiert.

Einleitung: Die Evidenz der sekundärpräventiven Effekte sportlicher Aktivität bei der koronaren Herzkrankheit (KHK) sind durch ausreichend viele Studien belegt (Bjarnason-Wehrens et. al., 2009). Eine kontinuierliche Fortführung der sportlichen Aktivitäten nach einer kardiologischen stationären Rehabilitation hätte insbesondere für sportlich inaktive Patienten einen hohen gesundheitlichen Nutzen und würde das Risiko der kardialen Sterblichkeit reduzieren (lestra et al., 2005). Verhaltensbezogene Interventionen mit motivationaler und volitionaler Ausrichtung haben ein gutes Potential, das Bewegungsverhalten nachhaltig zu verändern (Geidl et. al., 2012). Ziel: In der vorliegenden Untersuchung wurde das psychologische Interventionsprogramm MoVo-LISA erstmalig in einem RCT- Design bei 100 inaktiven kardiologischen KHK-Patienten im Rahmen eines stationären Rehabilitationsaufenthaltes evaluiert und auf seine Wirksamkeit und Effekte innerhalb der ersten sechs Wochen nach der Reha überprüft. Methoden: In einer klinischen, quantitativen, zweiarmigen, randomisierten und kontrollierten Interventionsstudie mit einem drei und sechs Wochen Follow-up wurden zwei Gruppen, Interventionsgruppe [= MoVo-LISA + Standard Behandlung] (ท=50) und Kontrollgruppe mit dem Standardprogramm der Klinik [„usual care“ ohne MoVo-LISA] (ท=50), verglichen. Zu zwei Messzeitpunkten wurden das Sport- und Bewegungsverhalten und an drei Messzeitpunkten psychologische Verhaltensparameter sowie Daten zur Lebensqualität und Herzangst per Fragebogen erhoben. Ergebnisse: Insgesamt konnten in der Längsschnittanalyse 70 Patienten (IG= 35) und (KG= 35) ausgewertet werden. Sechs Wochen nach der Rehabilitation zeigte sich in der IG ein mit (310 Min./wöchentl.) signifikant höherer Gesamtumfang der Sportaktivität gegenüber der Kontrollgruppe (139 Min./wöchentl.). In der Interventionsgruppe waren sechs Wochen post Rehabilitation 100% der Patienten sportlich aktiv, in der Kontrollgruppe hingegen nur 65,71 % der Patienten. Die Kontrollgruppe zeigte am Ende der Rehabilitation eine signifikant bessere Angina Pectoris-Stabilität im Vergleich zur Interventionsgruppe. Dieser Effekt war jedoch sechs Wochen nach der Rehabilitation nicht mehr vorhanden. Bei der Variable Herzangst wurden keine signifikanten Unterschiede zwischen den Gruppen festgestellt. In den psychologischen Variablen zeigten sich erste augenfällige Effekte bei der Planungstiefe, Zielintention und den positiven Konsequenzerwartungen, jedoch waren keine signifikanten Interventionseffekte vorhanden. Fazit: Um die Aussagekraft der Ergebnisse hinsichtlich einer Veränderung zu einem dauerhaft aktiven Lebensstil zu erhöhen, sollten weitere Untersuchungen mit einer stärkeren Stichprobengröße und längeren Nachbefragungszeiträumen (6 und 12 Monate post Rehabilitation) erfolgen. Introduction: Secondarily preventive effects of sports activities on coronary artery disease (CAD) have been documented in various studies (Bjarnason-Wehrens et. al., 2009). A regular continuation of sports activities after cardiological inpatient rehabilitation would be of particular sanitary use for lesser sportive patients and would reduce the risk of cardiac mortality (lestra et al., 2005). Behavior-connected interventions with motivational and volitional orientation have great potential to effectively change and influence exercise behavior (Geidl et. al., 2012). Objective: In the investigation at hand, the psychological intervention programme MoVo-LISA was evaluated for the first time in a RCT design with 100 inactive cardiological CAD patients during inpatient rehabilitation, and it was surveyed in terms of its effectiveness within the initial six weeks after rehabilitation. Methods: In a clinical, quantitative, two-fold, randomized and controlled intervention study with a follow-up of three and six weeks respectively, two groups were compared: an intervention group [MoVo-LISA + standard treatment] (ท = 50) and a control group with the hospital's standard Programm [“usual care“ without MoVo-LISA] (ท = 50). Sports and exercise behavior were measured twice, while data on psychological behavior parameters as well as on quality of life and Da Costa’s syndrome were collected via a questionnaire three times. Results: In total, the data of 70 patients could be analyzed in a longitudinal study (IG = 35 and KG = 35). Six weeks after rehabilitation, a significantly higher rate of sports activities (310 min/week) could be noted in comparison to the control group (139 min/week). Six weeks after rehabilitation, 100% of the intervention group patients were active, while this applied to only 65.71% in the control group. At the end of rehabilitation, the latter showed a significantly better stability for angina pectoris as compared to the intervention group. This effect, however, was gone six weeks after rehabilitation. The ‘Da Costa’s syndrome’ variable showed no significant differences between the groups. Among psychological variables, initial obvious effects could be noted for the action planning, goal intention and positive outcome expectations; however, no significant intervention effects could be detected. Conclusion: In order to enhance the validity of the results in terms of making a change towards a more active lifestyle, further studies should be conducted with a larger sample and longer periods of post-surveying (i.e. 6 and 12 months after rehabilitation). Diese Arbeit befasst sich mit dem Aufbau eines körperlich aktiven Lebensstils bei koronaren Herzpatienten. Die positiven Wirkungen von sportlicher Aktivität auf die bekannten Risikofaktoren der koronaren Herzkrankheit (KHK) sind in mehreren ausreichend großen, randomisierten Studien oder Metanalysen nachgewiesen (Bjarnason-Wehrens et al., 2009). Die langfristigen Ergebnisse der Sekundärprävention auf die lebensstilbedingten Risikofaktoren bei KHK- Patienten sind jedoch sehr ernüchternd. Ein guter Beleg hierfür sind die Ergebnisse der EUROASPIRE-Studien l,ll und III. Diese Untersuchungen zeigen eindrücklich, wie sich die kardiovaskulären Risikofaktoren bei Herzpatienten nach einer akutund rehamedizinischen Behandlung über einen längeren Zeitraum negativ entwickeln. In diesen Langzeitstudien ist eine Zunahme der kardiovaskulären Risikofaktoren bei den KHK- Patienten nachweisbar (Prugger et al., 2012). Trotz hoher Motivation gelingt es vielen Patienten nicht, ihre geplanten Absichten nachhaltig im Sinne einer Gewohnheit in den Alltag umzusetzen (Fuchs, 2007). Nur wenige Patienten schaffen es, im Anschluss an eine stationäre Rehabilitation Bewegungsaktivitäten fest in ihren Alltag zu integrieren (Wagner, 2007, S.71-88). Das am Anfang der Arbeit erwähnte Zitat von Erich Kästner: „ Es gibt nichts Gutes, außer man tut es“ ist im Hinblick auf mehr sportliche Aktivität im Alltag für körperlich inaktive KHK-Patienten eine große Herausforderung, da es eine Änderung der lieb gewordenen Gewohnheiten zur Folge hat. Fast jeder kennt sie, die guten Vorsätze, sein Verhalten zu verändern. Bei der Umsetzung ist schon fast jeder einmal gescheitert und hat zum Beispiel das regelmäßige Joggen wieder aufgegeben oder womöglich gar nicht erst damit angefangen (Scholz & Sniehotta, 2006). Dieses Phänomen des „Wollens“, aber das gewünschte Verhalten dann trotz des „Wollens“ nicht zu tun, ist in der gesundheitspsychologischen Literatur unter dem Namen „Intentions-Verhaltens-Lücke“ bekannt (Sheeran, 2002). Für die stationäre oder ambulante Rehabilitation stellen sich somit folgende Fragen: Von welchen Faktoren und Mechanismen hängt es ab, dass ein motivierter und zuvor sportlich inaktiver KHK-Patient nach der Rehabilitation dauerhaft und nachhaltig körperlich aktiv ist? Welche genauen Techniken und Interventionen fördern bei diesen Patienten nachhaltig die körperliche und sportliche Aktivität? Die Freiburger Forschergruppe um Reinhard Fuchs entwickelte zu dieser Problematik ein standardisiertes und ทาodualisiertes psychologisches Interventionsprogramm mit dem einprägsamen Namen MoVo-LISA. Das Akronym MoVo steht für Motivation und Volition, LISA für „Lebensstil-Integrierte-Sportliche- Aktivität“. Als theoretischen überbau orientiert sich die Intervention an dem MoVo- Prozessmodell. Kernbotschaft dieses Modells ist es, dass eine Verhaltensänderung nicht allein vom Ausmaß der Motivation abhängig ist. Neben der Motivation ist es entscheidend, inwieweit die Person in der Lage ist, das gewünschte Verhalten in eine ganz konkrete
Handlung umzusetzen. Das Interventionsprogramm ist sehr gut für das Setting der stationären Rehabilitation geeignet und soll inaktiven chronisch kranken Menschen helfen, einen körperlich aktiven Lebensstil aufzubauen und diesen im Anschluss an die Rehabilitation zu einem festen integrativen Bestandteil ihres Alltags zu machen. In einem quasi experimentellen Untersuchungsdesign wurde das Programm bereits erfolgreich an 220 orthopädischen Patienten im Kontext eines stationären Aufenthaltes evaluiert (Fuchs et al., 2010; Fuchs et al., 2011). Eine genauere und ausführliche Darstellung der Ergebnisse dieser bereits durchgeführten Evaluation des MoVo-LISA-Programmes erfolgt im Kapitel 2.5 dieser Arbeit. Trotz der eindrücklichen Effekte und Ergebnisse dieser Untersuchung gab es auch Einschränkungen in dieser Studie. Zur Vermeidung von Kontaminationseffekten unter den Patienten hat man sich damals für ein quasi-experimentelles Studiendesign entschieden, was automatisch eine Limitierung der internen Validität zur Folge hatte. Ganz bewusst hat man in der vergangenen Untersuchung auf eine zeitlich parallel geführte, randomisierte Zuweisung der Patienten in eine Kontroll- bzw. Interventionsgruppe verzichtet (Fuchs, et al., 2010). Die hier vorliegende Arbeit befasst sich mit der erneuten Evaluation des psychologischen Interventionsprogrammes MoVo-LISA im Kontext der stationären Rehabilitation. Hierzu wurde ein neuer Ansatz gewählt. Im Gegensatz zum quasi-experimentellen Design wurde jetzt ganz gezielt ein randomisiert-kontrolliertes Interventionsdesign ausgewählt, um die Einschränkungen der internen Validität der ersten Evaluation zu umgehen. Weiterhin wurde mit den KHK-Patienten eine andere und neue Zielgruppe von Patienten ausgewählt. In der vorliegenden Untersuchung wird somit das MoVo-LISA-Programm erstmalig in einem kontrolliert-randomisierten Design bei 100 inaktiven kardiologischen KHK-Patienten im Rahmen eines stationären Rehabilitationsaufenthaltes in den Kliniken „Lazariterhof“ und „Baden“ des Park-Klinikums in Bad Krozingen evaluiert und auf seine Wirksamkeit und Effekte innerhalb der ersten sechs Wochen nach einem stationären Rehaaufenthalt überprüft. Mit diesem Effektivitätsnachweis wäre ein wichtiger Schritt für den Übergang der Rehabilitationsphase II in die Rehabilitationsphase III erbracht.
Zu Beginn dieser Arbeit wird im Kapitel 2.1 zunächst die koronare Herzkrankheit (KHK) mit ihren Erscheinungsformen und den epidemiologischen und ätiologischen Aspekten der Erkrankung näher beschrieben. Hierbei wird insbesondere auf die mit der KHK-assoziierten Risikofaktoren eingegangen. Danach erfolgt im Abschnitt 2.2 der Arbeit eine ausführliche Darstellung der Bedeutung von körperlicher und sportlicher Aktivität für diesen Patientenkreis. Die sekundärpräventiven Effekte werden an Hand der aktuellen Studienlage dargestellt. Ebenfalls werden ganz konkrete Bewegungs- und Trainingsempfehlungen für die KHK- Patienten auf Grundlage der Empfehlungen der kardiologischen Fachgesellschaften aufgezeigt. Danach erfolgt eine Risiko-Nutzen-Abwägung der körperlichen Aktivität für KHK- Patienten. Um die Wichtigkeit und Notwendigkeit von Interventionen zur Förderung eines körperlich aktiven Lebensstils bei KHK-Patienten darzustellen, werden im Abschnitt 2.3 die Adhärenzdefizite im Kontext der kardiologischen Rehabilitation an Hand verschiedener Untersuchungen aufgezeigt. Im Kapitel 2.4 erfolgt die Darstellung der theoretischen Grundlagen von psychologischen Faktoren und Mechanismen, die für den Aufbau eines aktiven Lebensstils von Bedeutung sind. Hier werden der Motivationsbegriff, das Rubikonmodell und verschiedene gesundheitsfördernde Modelltheorien erläutert. Diese Ausführungen sind insbesondere für das Verständnis von volitionsorientierten Interventionsstrategien bedeutsam. Die allgemeintheoretischen Ausführungen der verschiedenen psychologischen Faktoren und Mechanismen sind wichtig für das tiefere Verstehen des Movo-Prozessmodells, das in diesem Kapitel ebenfalls sehr ausführlich erklärt und dargestellt wird. Ohne Kenntnisse des MoVo-Prozessmodells ist das in dieser Arbeit im Rahmen einer RCT-Studie bei KHK-Patienten auf Effektivität zu prüfende MoVo-LISA- Konzept nicht nachvollziehbar. Das Interventionskonzept wird detailliert beschrieben, damit sich der Leser eine theoretische Vorstellung von der durchgeführten Intervention machen kann. Im nachfolgenden Kapitel 2.5 werden bereits vorhandene evidenzbasierte Effektivitätsnachweise zur Steigerung der körperlichen Aktivität an Hand von verschiedenen Studien aufgezeigt. Für den Kontext dieser Arbeit thematisch relevant sind hierbei insbesondere Untersuchungen mit volitionstheoretischem Ansatz und solche die in Deutschland im rehabilitativen Kontext durchgeführt wurden. Ebenfalls berücksichtigt wurde eine internationale Metaanalyse. In diesem Kapitel findet der Leser auch die bereits angesprochenen Ergebnisse und Erläuterungen zu der erfolgreichen MoVo-LISA- Intervention bei orthopädischen Patienten mit dem quasi-experimentellen Untersuchungsdesign. Abschließend wird im Kapitel 2.6 der gesamte theoretische Hintergrund und Forschungsstand noch einmal kurz zusammengefasst, und die Interventionsstudie inhaltlich begründet. Das Kapitel drei beschreibt sehr ausführlich das Design und die methodische Vorgehensweise der MoVo-LISA-RCT-lnterventionsstudie bei inaktiven KHK-Patienten mit allen wichtigen Parametern wie Zielsetzung, Fragestellung, Stichprobe Interventions- und Messdesign sowie die Hauptzielgrößen mit Messinstrumenten und das statistische Vorgehen. Im Kapitel vier werden die Ergebnisse meiner Untersuchung dargestellt, die dann abschließend in Kapitel fünf kritisch diskutiert werden.
Die koronare Herzkrankheit, kurz KHK, entsteht durch eine Verengung der Herzkranzarterien (Koronararterien) aufgrund einer fortschreitenden Arteriosklerose (Fischbach & Keithahn, 2009). Sie ist definiert als eine Erkrankung der Koronargefäße, welche durch Arteriosklerose verändert bzw. geschädigt sind (Rost, 2005, S.234). Die Gefäßverengungen entstehen durch sogenannte arteriosklerotische Plaques. Dies sind Fett- und Kalkablagerungen an den Innenwänden der Herzkranzgefäße (Fischbach & Keithahn, 2009, S.197). Die Entstehung der Arteriosklerose ist ein schleichender und fortschreitender Prozess, der über viele Jahre bis Jahrzehnte andauert. Der arteriosklerotische Prozess mit den entsprechenden Plaquebildungen ist in der folgenden Abbildung dargestellt.

Dieser Prozess ist sehr komplex und kann schon lange vor dem Eintreten klinischer Symptome beginnen (Hoberg, 2007, S.166). Schreitet die koronare Arteriosklerose der Herzkranzgefäße ohne ein akutes Ereignis fort, wird die Erkrankung erst dann symptomatisch, wenn es zu einer sogenannten Ischämie des Herzmuskels kommt. (Rost, 2005, S.234). Die Ischämie ist eine Mangeldurchblutung des Myokards und entsteht bei einem unzureichenden Sauerstoffangebot für den Herzmuskel über die Koronararterien im Verhältnis zu dem benötigten Sauerstoffbedarf des Herzmuskels (Gabriel, et al., 2007, S. 155). In Abhängigkeit vom Ausmaß der Arteriosklerose kann der Herzmuskel bereits in Ruhe oder erst bei körperlicher Belastung und psychischer Erregung nicht mehr ausreichend durchblutet werden. Die aus der Ischämie resultierende Minderversorgung des Herzmuskels mit Sauerstoff nennt man koronare Insuffizienz. Diese verursacht als klinisches Leitsymptom einen typischen anfallsartigen Brustschmerz mit Engegefühl, die sogenannte Angina pectoris (Fischbach & Keithahn, 2009, S. 197). Diese typischen Schmerzen können in den linken Arm, die Schultern, den Hals, die Wangen und in die Zähne ausstrahlen (Gabriel, et al., 2007, S. 156). Man unterscheidet zwischen der stabilen Angina pectoris und der instabilen Angina pectoris. Bei der stabilen Angina pectoris treten die Anfälle nicht in Ruhe, sondern unter bestimmten körperlichen Belastungen, zum Beispiel beim Sport oder Treppensteigen auf. Bei einer instabilen Angina pectoris treten die Anfälle auch unter Ruhebedingungen und gehäuft ohne erkennbaren Anlass auf (Rost, 2005, S.235). Die instabile Angina pectoris, der Herzinfarkt und der plötzliche Herztod werden als akutes Koronarsyndrom bezeichnet. Sie zählen zu den häufigsten Erscheinungsformen der KHK (Hoberg, 2007, S. 166). Auch wenn der arteriosklerotische Prozess normalerweise langsam verläuft, kann durch ein Einreißen oder Aufbrechen einer kleinen Plaque, die noch nicht zu einer Stenose oder Beeinträchtigung der Durchblutung geführt hat, ein akuter Koronargefäßverschluss entstehen (Gielen, 2010). Der Patient muss vorher keine Symptome für eine Herzkranzgefäßerkrankung gezeigt haben. Der akute Gefäßverschluss entsteht auf Grund einer schnell einsetzenden Blutgerinnung (Thrombose), wenn das fließende Blut mit einer aufgebrochenen arteriosklerotischen Plaque in Kontakt kommt (Hoberg, 2007, S. 166). Einen solchen akuten Gefäßverschluss inklusive seiner Folgen bezeichnet man als Herzinfarkt. Ein Myokardinfarkt wird in zwei unterschiedliche Formen unterteilt. Ist eine ST-Streckenhebung im EKG nachweisbar, bezeichnet man diese Form als ST-Streckenhebungsinfarkt, kurz STEMI genannt. Einen Infarkt ohne einen ST-Hebungsnachweis im EKG bezeichnet man als Nicht-ST- Streckenhebungsinfarkt, kurz NSTEMI genannt (Hoberg, 2007, S. 166). Zur besseren Veranschaulichung der Differenzierung des akuten Koronarsyndroms mit seinen Erscheinungsformen dient die Tabelle eins auf der folgenden Seite.

Die meisten Patienten werden auf Grund von klinischen Beschwerden in Form eines akuten Koronarsyndroms identifiziert. Nach entsprechender akutmedizinischer Behandlung und Therapie gehen sie in einen chronischen, stabilen Krankheitszustand über (Gabriel et al., 2007, S. 155). Die typischen klassischen Symptome der sogenannten Angina pectoris lassen nach akutmedizinischer Behandlung (Revaskularisierung des Herzmuskels mittels PTCA, Stent oder Bypass-Operation) deutlich nach, da unter Ruhebedingungen wieder eine ausreichende Sauerstoffversorgung der Herzmuskelzelle gewährleistet ist (Gabriel et al., 2007, S. 156). Bei erhöhtem Sauerstoffbedarf unter Belastungsbedingungen, bei der es zu einer Erhöhung von Herzfrequenz, Kontraktilität und Wandstress kommt, kann jedoch das Gleichgewicht zwischen Sauerstoffbedarf und Zufuhr evtl, nicht mehr aufrecht erhalten werden. In solch einem Falle kann es wieder zu symptomatischen Beschwerden in Form der beschriebenen Angina pectoris-Problematik kommen (Gabriel et al., 2007, S. 156). Die stabile Angina pectoris kann nach der CCS-Klassifikation (Klassifikation der Canadian Cardiovascular Society) in verschiedene Schweregrade eingeteilt werden (siehe Tabelle zwei auf der folgenden Seite).

Erfreulicherweise ist in Deutschland ein Rückgang der Sterblichkeit auf Grund von HerzKreislauf-Erkrankungen zu verzeichnen (Gößwald et al., 2013, S.650). Jedoch stellen die kardiovaskulären Erkrankungen weiterhin trotz rückläufiger Mortaliätsraten immer noch laut der Gesundheitsberichterstattung des statistischen Bundesamtes aus dem Jahre 2012 mit 41,1% und insgesamt 349.212 sterbefällen die häufigste Todesursache in Deutschland dar. Hierbei liegt die KHK-bedingte Sterblichkeit mit 14,7% und insgesamt 128.171 sterbefällen an der Spitze der kardiovaskulären Mortalitätsfälle (https://www.destatis.de/DE/ZahlenFakten/ GesellschaftStaat/Gesundheit/Todesursachen/Tabellen/EckdatenTU.html, am 02.04.2014). Jeder sechste Todesfall in Deutschland ist demnach durch die koronare Herzkrankheit verursacht (Debus et al., 2013, S.645). Der Rückgang der Sterblichkeit liegt vor allem an den verbesserten therapeutischen, medikamentösen und diagnostischen Möglichkeiten in der Akutbehandlung des Herzinfarktes (Gößwald, 2013, S. 650). Rückläufige Sterblichkeitsfälle der kardiovaskulärbedingten Erkrankungen bedeuten jedoch nicht zwangsläufig eine Abnahme der Erkrankungsfälle und Gesundheitskosten in diesem Bereich. Ganz im Gegenteil, gerade die Zunahme der überlebensrate nach Herzinfarkt verbunden mit einer höheren allgemeinen Lebenserwartung führt zu einer wachsenden Bedeutung der Sekundärprävention im Bereich der kardiovaskulären Erkrankungen (Löwel & Meisinger, 2006). Der überlebte Herzinfarkt erhöht die Lebenszeitprävalenz einer koronaren Herzerkrankung. Der Anteil der Personen, die mit sekundärpräventiven Maßnahmen wie spezifischen Therapien, rehabilitativen oder pflegerischen Angeboten versorgt werden müssen steigt hierdurch an. Die Herzinfarktsprävalenz ist sehr stark vom Geschlecht und dem Alter abhängig. Insgesamt 7% der Frauen und 10% der Männer geben nach dem Gesundheitsbericht des Bundes aus dem Jahr 2012 an, dass bei Ihnen eine KHK-Erkrankung diagnostiziert wurde (Debus et al., 2013). In den Altersgruppen bis zum 44. Lebensjahr weisen beide Geschlechter sehr niedrige Prävalenzraten auf. Frauen haben hier Werte von unter einem Prozent und Männer Werte bis zu zwei Prozent. Mit zunehmendem Alter (45-64 Jahre) kommt es bei den Frauen zu einem Anstieg der diagnostizierten Fälle auf 4 % und bei den Männern auf 10%. In der Altersgruppe der 65-Jährigen steigt die Erkrankungshäufigkeit bei Frauen auf 18% und bei Männern auf 28% an (Debus et al., 2013). Diese hohen, altersbedingten Prävalenzraten spiegeln sich auch in der deutschen Krankenhausstatistik wieder. Im Jahr 2005 wurden in Deutschland allein 737.468 Patienten wegen einer KHK stationär aufgenommen (Damm et al., 2011). Nach überstandenem Herzinfarkt erleiden bis zu 20% der Männer und 35 % der Frauen innerhalb von 6 Jahren einen Reinfarkt (Damm et al., 2011, S. 122). Die koronare Herzkrankheit (KHK) verursacht mehr Folgeerkrankungen und größere wirtschaftliche Kosten als jede andere Krankheit in den entwickelten Industrienationen (Niederseer & Seebauer, 2010). Untersuchungen des Robert-KochInstitutes zeigen, dass den Krankenkassen im Jahr 2006 etwa 35,2 Millarden Euro direkte Kosten im Zusammenhang mit kardiovaskulären Erkrankungen entstanden sind. Der Anteil der koronaren Herzerkrankung lag hier bei etwa 7 Milliarden Euro direkten Kosten (Damm et al., 2011). Unter dem Begriff der direkten Kosten werden diejenigen Ausgaben verstanden, die zur Erbringung einer medizinischen Leistung erforderlich sind. Neben den direkten Kosten spielen die sogenannten indirekten Kosten eine wesentliche Rolle bei den kardiovaskulären Erkrankungen. Zu den indirekten Kosten zählen die durch KHK-Erkrankung entstehenden Folgen wie Invalidität durch Herzinsuffizienz, Frühberentung oder vorzeitigen Tod. Der dadurch verursachte volkswirtschaftliche Schaden bringt enorme Produktivitätsverluste mit sich (Damm et al., 2011). Untersuchungen des Robert-Koch-Institutes haben gezeigt, dass im Jahr 2009 rund 127.000 Erwerbstätigkeitsjahre aufgrund der KHK-Erkrankung verloren wurden (Lange, 2011). Legt man hierein durschnittliches Arbeitnehmerentgeld von 34.100 Euro jährlich zu Grunde, so errechnet sich ein volkswirtschaftlicher Produktionsausfall von etwa 4,3 Milliarden Euro aufgrund der KHK-Erkrankungen im Bereich der Fehlzeiten (Damm et al., 2011, S.125). Insgesamt zeigen die epidemiologischen Daten, dass die koronare Herzkrankheit und ihre Folgen eine besondere Herausforderung für die Medizin und das Gesundheitssystem darstellen. Primäre und auch sekundärpräventive Strategien im Bereich der Diagnostik, Therapie und Behandlung müssen optimiert und weiterentwickelt werden.
Die Entstehung der KHK ist ein komplexer Mechanismus, der seine Ursache in der Arteriosklerose der Koronarkranzgefäße hat (Gabriel et al., 2007, S. 155). Die Arteriosklerose ist eine durch entzündliche Prozesse hervorgerufene Ansammlung von sogenannten Plaques in den Innenwänden der Arterien. In diesen Plaques konnten 30-mal mehr cholesterinhaltige Substanzen entdeckt werden, als in normalen Geweben üblich. Die Theorie, dass zirkulierende Lipide an der Entstehung von diesen Ablagerungen beteiligt sind, gilt heute als unzweifelhaft und gesichert (Vance & Van den Bosch, 2000). Nicht der Stenosegrad, sondern die Stabilität der ateriosklorotischen Plaques ist ein entscheidender Faktor für die Prognose eines akuten Koronarsyndroms. Aus Untersuchungen ist bekannt, dass 68% aller Infarkte auf Koronarstenosen zurückzuführen sind, die einen Stenosegrad von weniger als 50 Prozent aufweisen. Gerade instabile Plaques mit einem hohem Lipidanteil und Makrophagengehalt führen eher zu einem Infarkt als kollagenreichere Plaques (Gielen, 2010). Um die Ätiologie der KHK in ihrer Gesamtheit und Komplexität richtig zu deuten, ist es bedeutsam zu erwähnen, dass die Arteriosklerose eine systemische Erkrankung ist. Sie betrifft den gesamten Gefäßbaum und nicht nur die Herzkranzgefäße (Espinola-Klein, 2002, S.221). Untersuchungen aus der Pathologie konnten sehr eindrücklich zeigen, dass häufig neben der bereits klinisch bekannten arteriosklerotischen Grunderkrankung noch weitere Regionen betroffen waren (Espinola-Klein, 2002, S.221). Dieser Zusammenhang konnte an bereits herzkatherisierten Patienten durch zusätzliche Diagnostik in Form von Duplexsonographie der Karotiden oder Dopplersonographie der Beinarterien bestätigt werden (Espinola-Klein, 2002, S.221). Demnach erklärt sich auch das erhöhte Risiko von KHK-Patienten, mit einer Wahrscheinlichkeit von 20% innerhalb von 10 Jahren einen Schlaganfall zu erleiden (Kännel, 1994). In der Framingham-Studie konnten folgende Risiokfaktoren als schädlich für die Endothelzellen der Gefäße ermittelt werden: Hyperlipidämie, Rauchen, Hypertonie, Diabetes, Adipositas und körperliche Inaktivität (Gabriel et al., 2007). Die familiäre und genetische Disposition konnte ebenfalls aus der epidemiologischen Framingham-Studie nachgewiesen werden. Eine genetische Disposition liegt immer dann vor, wenn in der Verwandtschaft 1. Grades eine KHK - Erkrankung vorliegt. Bei männlichen Verwandten 1. Grades muss die KHK vor dem 55. Lebensjahr, bei weiblichen Verwandten muss die KHK vor dem 65. Lebensjahr diagnostiziert worden sein (Erdmann & Schunkert, 2007, S.316). Der Einfluss eines erhöhten Homocysteinspiegels auf verschiedene Manifestationen der Arteriosklerose ist ebenfalls untersucht und nachgewiesen. Eine Hyperhomocysteinämie korreliert demnach mit der Arterioskleroseerkrankung (Boushey et al., 1995). Der arteriosklerotische Prozess ist abhängig vom Ausmaß und von der Anzahl der Risikofaktoren. Interessanterweise besitzen die genannten Risikofaktoren eine unterschiedliche Wertigkeit bezüglich der Lokalisation der Arteriosklerose. Einen guten Überblick über die unterschiedlichen Bedeutungen einzelner Risikofaktoren bezüglich der einzelnen Gefäßregionen gibt die nachfolgende Abbildung zwei.
Für die Koronargefäße ist demnach die Hyperlipidämie der wichtigste Risikofaktor, gefolgt vom Rauchen, Hypertonie und dem Diabetes mellitus (Espinola-Klein, 2002, S.223).

Trotz der Gewichtung einzelner kardiovaskulären Risikofaktoren existieren die genannten Faktoren nicht unabhängig voneinander, sondern verstärken sich synergistisch und erhöhen das kardiovaskuläre Risiko kontinuierlich (Gohlke 2007; Gabriel et al., 2007, S. 155; Rost, 2005,S. 143 ff). Zur Bestimmung eines individuellen kardiovaskulären Risikos werden Methoden angewandt, die der Vielzahl der Risikofaktoren Rechnung tragen und die das Risiko als kontinuierliche Größe quantifizieren. Dazu sind sogenannte Scoresysteme geeignet (Schneider, 2009, S.2). Bei der scorebasierten Risikostratifikation müssen primärpräventive Scores und sekundärpräventive Scores differenziert werden. Für Patienten nach einem ST-Streckenhebungsinfarkt (STEMI) hat sich der TIMI-Risiko-Score zur Beurteilung eines Sterblichkeitsrisikos innerhalb von 30 Tagen bewährt (Gohlke, 2007). Die in Deutschland verwendeten Risikoscores für die Risikoeinschätzung koronarer Ereignisse von noch nicht erkrankten Personen sind der PROCAM-Score, der Framingham-Score und der ESC-Score (Silber et al., 2008, ร,638). Auf eine tiefergehende, ausführliche Darstellung der etablierten Scores wird in dieser Arbeit verzichtet, da dies für das Gesamtverständnis der Thematik nicht von erhöhter Relevanz ist. Bedeutsamer im Kontext dieser Arbeit ist es, die Unterteilung der jeweiligen Risikofaktoren in bestimmte Klassen vorzunehmen. Man unterscheidet,, beeinflussbare Risikofaktoren“ von „ nicht beeinflussbaren Risikofaktoren“ mit oder ohne prognoseverbessernden Effekten auf zukünftige kardiovaskuläre Ereignisse (Schneider, 2009, S.2). Beeinflussbar und prognostisch wirksam bedeutet in diesem Zusammenhang, dass der Patient selbst übereine entsprechende Lebensstilmodifikation und durch die gezielte Einnahme von wirksamen und richtig dosierten Medikamenten einen positiven Einfluss auf seine individuellen Risikofaktoren nehmen kann. Nach Schneider (2009, S.2) lassen sich die Risikofaktoren in drei unterschiedliche Klassen aufteilen, die im Folgenden dargestellt werden:
1. Beeinflussbare Risikofaktoren mit prognostisch wirksamen Effekten
Diese können durch entsprechende Lebensstilmodifikationen oder Medikamente nachhaltig beeinflusst werden und zu einer Reduktion des kardiovaskulären Risikopotentials führen. Hierzu zählen: Verzicht auf Nikotinkonsum, Senkung des Hypercholesterins und der arteriellen Hypertonie, Vermeidung von Diabetes mellitus, Steigerung des HDL-Cholesterins, Senkung der Triglyzeride, Vermeidung von Übergewicht und Adipositas und die Erhöhung der körperlichen und sportlichen Aktivität.
2. Beeinflussbare Risikofaktoren mit prognostisch nicht wirksamen Effekten
Diese Risikofaktoren sind zwar beeinflussbar, haben jedoch prognostisch einen wenig bedeutsamen Effekt. Hierzu zählt die Senkung des erhöhten Homocysteinspiegels durch entsprechende Medikamente.
3. Nicht beeinflussbare Risikofaktoren
Bei diesen Faktoren hat man weder durch eine Lebensstilmodifikation noch durch entsprechende Medikamente die Möglichkeit Einfluss auf das kardiovaskuläre Risikoprofil zu nehmen. Hierzu zählen: das Alter, das Geschlecht und die familiäre oder genetische Disposition.
Betrachtet man die dargestellte Klassifizierung genauer, so ist festzustellen, dass die beeinflussbaren Risikofaktoren den größten Teil des Risikoprofils ausmachen. Somit ist es verständlich und nachvollziehbar, dass die medikamentöse Versorgung und die Hinführung und Unterstützung zu individuellen Lebensstilmodifikationen wesentliche und zentrale Bausteine in der Therapie der KHK-Patienten sind. Die sekundärpräventive Bedeutung einer medikamentösen Therapie kombiniert mit einer konsequenten Lebensstilmodifikation bezüglich der körperlichen Aktivität (täglich 30 Minuten Ausdauertraining) konnte in der COURAGE-Studie eindrücklich dargestellt werden (Gielen, 2010). In dieser prospektiv- randomisierten Studie wurde an KHK-Patienten eine konsequente konservative Therapie, bestehend aus Medikamenten und täglichem Ausdauertraining, mit der standardmäßig durchgeführten akutmedizinisch invasiven Methode des Herzkatheters (PTCA) auf die Endpunkte Mortalität und nicht tödliche Myokardinfarkte verglichen. Die medikamentöse Therapie kombiniert mit konsequenter Lebensstilmodifikation (täglich 30 Minuten Ausdauertraining) war im Vergleich zur PTCA bezüglich der Reduktion von kardiovaskulären Ereignissen deutlich effektiver (Boden et al., 2007).
Die Begriffe körperliche Aktivität und Training werden im gesundheitsbezogenem Verständnis häufig synonym verwandt. Im Zusammenhang mit den trainings- und bewegungstherapeutischen Maßnahmen und Empfehlungen bei einer koronaren Herzerkrankung ist es jedoch wichtig, diese Begrifflichkeiten zum besseren Verständnis gegeneinander abzugrenzen. Demnach wird körperliche Aktivität als jede durch die Skelettmuskulatur hervorgebrachte Bewegung definiert, die zu einem substanziellen Anstieg des Energieverbrauchs über den Ruhewert hinaus führt (Samitz, 2002). Hingegen versteht man unter Training einen Teilbereich der körperlichen Aktivität, der geplant, strukturiert, wiederholt und zielgerichtet zur Verbesserung der körperlichen Leistungsfähigkeit eingesetzt wird (Samitz, 2002). Training ist immer ein gesteuerter Prozess, bei dem eine individuell dosierte Belastung in Form von mehreren, strukturierten, auf einander abgestimmten Trainingseinheiten über einen bestimmten Zeitraum durchgeführt wird (Niederseer & Niebauer, 2010). Der in dieser Arbeit eingesetzte Fragebogen zur Erfassung der Bewegungsund Sportaktivität (kurz: BSA-Fragebogen) basiertauf einer begrifflichen Unterscheidung von Bewegungsaktivität und Sportaktivität (Fuchs et al., 2013). Nach Fuchs et al. (2013) sind, von der sehr weit gefassten Definition der körperlichen Aktivität ausgehend, die Bewegungs- und Sportaktivitäten jeweils Teilmengen der körperlichen Aktivität. Mit Bewegungsaktiväten sind Aktivitäten gemeint, die sehr gut in den Alltag integrierbar sind und die ausgeübt werden, um Aufgaben im Alltag zu erledigen. Häufig werden in diesem Zusammenhang das Zurücklegen möglichst vieler Strecken zu Fuß oder mit dem Fahrrad, die Benutzung von Treppen an stelle von Rolltreppen und Aufzügen oder auch körperliche Tätigkeiten im Haushalt und Garten aufgeführt (Fuchs et al., 2013, Niederseer & Niebauer, 2010). Zu den Sportaktivitäten zählen nach Fuchs et al. (2013) alle körperlichen Aktivitäten mit erhöhtem Energieverbrauch, die wir um ihrer selbst willen oder aus gesundheitlichen oder sozialen Gründen ausüben. Sportarten, bei denen das Leistungs- und Wettkampf motiv im Fokus steht, werden als „ Sportaktivitäten im engeren Sinne“ bezeichnet. Von „Sportaktivitäten im weiteren Sinne“ spricht man, wenn Motive wie Gesundheit, Wohlbefinden, Naturerlebnis und Sozialkontakt im Vordergrund stehen (Fuchs et al., 2013). Ausgehend von den dargestellten begrifflichen Unterscheidungen ist es bedeutsam zu erwähnen, dass die folgenden evidenzbasierten Effektivitätsnachweise des Kapitels 2.2.2 und Empfehlungen des Kapitels 2.2.3 bezüglich der körperlichen Aktivität bei einer KHK größtenteils auf der Grundlage von sportlichen Aktivitäten mit eindeutigen Trainingsparametern beruhen.
Ein wichtiges Ziel trainingsbasierter Maßnahmen in der Sekundärprävention und Therapie von KHK- Patienten ist es, den Verlauf und die Prognose der Erkrankung günstig zu beeinflussen (Bjarnason-Wehrens et al., 2009). Bei der KHK wird durch ein gezieltes körperliches Training ein günstiger prognostischer sekundärpräventiver Effekt auf die kardiovaskulären Risikofaktoren erzielt. Durch entsprechende Untersuchungen konnten wesentliche und entscheidende Wirkmechanismen der sportlichen Aktivität auf die KHK- Erkrankung identifiziert werden. Hierzu zählen: Die Verbesserung der koronaren Endothelfunktion, die Reduzierung der mechanischen Herzarbeit, die Verringerung der Progression manifester Koronarläsionen, eine Zunahme der Kollateralisierung und eine Reduktion des thrombogenen Risikos.Auf eine ausführliche Erklärung und Darstellung der Wirkmechanismen soll an dieser stelle der Arbeit verzichtet werden. Eine gute Übersicht hierzu findet man bei Bjarnason-Wehrens et al. (2009, S.28). Durch gezielte sportliche Aktivitäten können die bekannten kardiovaskulären Risikofaktoren: Hypertonie, Typ-2- Diabetes mellitus, Fettstoffwechsel-Störungen, abdominelle Adipositas und das metabolische Syndrom reduziert und günstig beeinflusst werden. Obwohl die Einzeleffekte jeweils moderat sind, dürfte die Kombination der Effekte ausschlaggebend sein (Bjarnason-Wehrens et al., 2009). Neben der Risikominimierung dient insbesondere das Ausdauertraining der Verbesserung der kardiopulmonalen Belastbarkeit des Patienten. Die prognostisch günstigen Effekte bei einer bestehenden Koronaren Herzerkrankung durch sportliche Aktivitäten im Sinne der Sekundärprävention sind wissenschaftlich ausreichend untersucht und nachgewiesen. Die Evidenz und damit der Nachweis der Wirksamkeit von körperlichem Training bei der KHK ist nach der Klassifizierung der European Society of Cardiology als sehr hochgradig (Evidenzgrad la) einzustufen. In mehreren ausreichend großen, randomisierten Studien oder Metaanalysen konnte insbesondere beim Ausdauertraining die Wirksamkeit belegt werden (Bjarnason-Wehrens et al., 2009). Langfristig durchgeführtes aerobes Ausdauertraining hat positive Effekte auf die bereits genannten Risikofaktoren. Bei richtiger Dosierung und Trainingssteuerung sowie gut eingestellter Hypertonie ist auch Krafttraining ein gutes Therapeutikum für KHK-Patienten. Gerade Patienten, die nach größeren herzchirugischen operativen Eingriffen lange bettlägerig waren, sind muskulär stark geschwächt. Ein zusätzlich zum Ausdauertraining durchgeführtes Krafttraining ist für Herzpatienten trainingswirksam und über eine verbesserte Muskelkraft kann richtig dosiertes Widerstandstraining KHK-Patienten zu mehr Lebensqualität verhelfen (Wonisch, 2009). Durch entsprechende Studien lässt sich belegen, dass Krafttraining auch beim Koronarpatienten zu signifikanten muskulären sowie hämodynamischen und kardio- respiratorischen Adaptionen führt (Wonisch, 2009). Körperliche Inaktivität bei KHK- Patienten ist mit einem erhöhten Mortalitätsrisiko verbunden. Im Sinne einer Risikoreduktion kann durch gezielte Trainingsinterventionen auf das kardiovaskuläre Mortalitätsrisiko der KHK-Patienten und auf die bekannten Risikofaktoren der koronaren Herzkrankheit Einfluss genommen werden. (Bjarnason-Wehrens et al., 2009). Im Folgenden werden die Aussagen bezüglich der Wirksamkeit von sportlichen Aktivitäten bei der koronaren Herzerkrankung und ihre sekundärpräventiven Effekte im Hinblick auf die Reduzierung der Risikofaktoren näher spezifiziert.
Mortalität:
In einer sehr umfangreichen Metaanalyse der Cochrane Library of Evidence based Medicine wurden 51 Studien und 8440 KHK Patienten untersucht. Die Autoren konnten in ihrer Untersuchung folgende Ergebnisse herausarbeiten: Bei Patienten mit einer koronaren Herzkrankheit wird die Gesamtmortalität durch regelmäßige körperliche Aktivität um 27% reduziert, das Risiko eines kardialbedingten Todesfalles kann durch sportliche Aktivitäten um 31% reduziert werden (Joliffe et al., 2001). Eine jüngere Metaanalyse aus dem Jahre 2005 untersuchte 63 Studien und 21295 KHK-Patienten und konnte dem körperlichen Training eine 25%-ige Risikoreduktion bezüglich der Sterblichkeitsfälle attestieren (lestraetal., 2005). Sehr gut untersucht ist ebenfalls der Zusammenhang einer guten kardiovaskulären Leistungsfähigkeit von KHK-Patienten und der Abnahme der kardialen Sterblichkeit. Eine durch Ausdauertraining verbesserte und durch eine spiroergometrische Messung erhobene relative Sauerstoffaufnahmefähigkeit (V02max) von 1,0 ml/kg/min ist mit einer Abnahme der kardialen Sterblichkeit von 8-10% assoziiert (Kavanagh et al., 2002; Kavanagh étal., 2003).
Hypertonie:
In einer Metaanalyse aus 44 randomisierten kontrollierten Studien und insgesamt 2674 Personen (normotensiv oder hypertensiv) hatte aerobes Ausdauertraining im Mittel auf den Blutdruck folgende Effekte: Signifikante Reduktion des systolischen Blutdrucks um -3,4 mmHg und des diastolischen Blutdrucks um -2,4 mmHg. Der blutdrucksenkende Effekt bei dem hypertensiven Patientenkollektiv war mit-7,5mmHg systolisch und 5,4mmHg diastolisch noch deutlich ausgeprägter (Fagard, 2005; Fagard, 2006). Interessanterweise ist der blutdrucksenkende Effekt unabhängig von der gewählten Trainingsintensität, sowohl bei geringen Intensitäten (40% der maximalen Ausdauerleistungsfähigkeit) wie auch bei höheren Intensitäten (80% der maximalen Ausdauerleistungsfähigkeit) konnten vergleichbare blutdrucksenkende Effekte erzielt werden (Fagard, 2005; Fagard 2006). Neben dem Ausdauertraining führt ein niedrig bis moderat dosiertes Krafttraining zu einer Reduktion des Blutdrucks. In mehreren Untersuchungen konnten signifikante, krafttrainingsbedingte
Blutdrucksenkungen von -3,2 mmHg systolisch und -3,5 mmHg diastolisch nachgewiesen werden (Fagard & Cornelissen, 2007; Fagard, 2006; Cornelissen & Fagard, 2005).
Hyperlipidämie:
In einer Metaanalyse konnte der Effekt von körperlichem Training als alleinige Intervention als nicht wirksam bezüglich der Höhe der LDL-Cholesterinkonzentration dargestellt werden (Kelley et al., 2006). Somit beeinflusst körperliches Training als alleinige Maßnahme die Höhe der LDL-Gesamtkonzentration nicht. Andere Untersuchungen geben jedoch erste Hinweise darauf, dass der Anteil der kleinen/ dichten LDL-Partikel gesenkt wird (Halverstadt, 2007). Aerobes Ausdauertraining führt hingegen zu einer signifikanten HDL- Zunahme (Kelly et al., 2006; Kodama et al., 2007). Weiterhin konnte die signifikante Reduktion der Triglyceride nachgewiesen werden ( Kelly et al., 2006).
Adipositas und Metabolisches Syndrom
Das Metabolische Syndrom wird über die Kriterien Taillenumfang (abdominelle Adipositas), Blutglukosespiegel, Blutdruck und Blutfettspiegel (Triglyceride und HDL-Cholesterin) definiert. Nach der American Heart Association liegt ein Metabolisches Syndrom vor, wenn 3 von 5 Kriterien erfüllt sind (Grundy et al., 2004). Zwischen dem Metabolischen Syndrom und der kardiovaskulären Morbidität und Mortalität besteht ein enger Zusammenhang. Lakka et al. (2002) konnten diesen Zusammenhang eindrücklich in einer 11-jährigen Beobachtungsstudie an 1209 finnischen Männern darstellen. Der abdominellen Adipositas kommt als Risikofaktor für eine KHK- Erkrankung eine herausragende Bedeutung zu. Die beste derzeitige Datenlage hierzu ist die INTER-HEART-Studie. Dort war für die Entstehung eines Myokardinfarkts die abdominelle Adipositas als bedeutsamster Risikofaktor identifiziert worden (Yusuf et al., 2004). Eine 10-jährige Beobachtung von 2541 Herzinfarktpatienten zeigte ebenfalls einen klaren Zusammenhang zwischen dem BMI und der Inzidenz eines Re-Infarkts (Rea, Heckbert & Kaplan, 2001). In diesem Zusammenhang ist es wichtig festzustellen, dass gerade körperliche Inaktivität und eine niedrig ergometrisch gemessene Ausdauerleistungsfähigkeit mit einem deutlich höheren Risiko verbunden ist, ein metabolisches Syndrom zu entwickeln (Grundy et al., 2004). Eine vermehrte körperliche Aktivität jedoch hat wiederum positive Auswirkungen auf das Gewicht und alle weiteren Parameter des Metabolischen Syndroms. Eine gute körperliche Leistungsfähigkeit reduziert das kardiovaskuläre Risiko erheblich, auch bei Patienten mit erhöhten BMI (Katzmarzyk, Church & Blair, 2004).
Insulinsensitivität/ Tvp-2-Diabetes
Ausdauertraining und Krafttraining führen zu einer Verbesserung der Insulinsensitivität. Dies wird durch eine entsprechende Zunahme der Insulinrezeptoren und eine verbesserte
Insulinbindung an die Insulinrezeptoren unter Erhöhung entsprechender Enzyme erreicht ((ว'Gorman et al., 2008; Karlsson, 2001). Interessanterweise scheinen diese positiven Auswirkungen nicht von der Trainingsintensität abhängig zu sein. In zwei voneinander unabhängigen Studien konnte gezeigt werden, dass niedrige Intensitäten ebenso effektiv sind wie höhere Intensitäten (Hansen et al., 2009 ; Nicklas et al., 2009). Bei Personen mit einer gestörten Glukosetoleranz kann die Manifestation eines Typ-2-Diabetes mellitus durch vermehrte körperliche Aktivität und Bewegung signifikant reduziert werden (Knowler et et al., 2002). Die Kombination von diätischen Maßnahmen und einer erhöhten Zunahme an sportlichen Aktivitäten ist jedoch in ihrer Wirkung hochgradig allen anderen Maßnahmen überlegen. In einer Metaanalyse konnte gezeigt werden, dass die Kombination von Bewegung und Diätetik deutlich effektiver ist als Sport oder Ernährungsmaßnahmen als Einzelintervention (Orozco et al., 2008).
Auf Basis der im Abschnitt 2.2.2 dargestellten wissenschaftlich nachgewiesenen Effekte von körperlicher Aktivität in Form von Ausdauer- und Krafttraining bei der koronaren Herzerkrankung werden in diesem Abschnitt ganz konkrete Trainingsempfehlungen für KHK- Patienten aufgezeigt. Wie bei jedem Medikament ist die gewünschte Wirkung von der richtigen Dosierung abhängig. Dies gilt selbstverständlich auch für das „Medikament Bewegung“ (Wagner & Becker, 2008). Für die Dosis- Wirkung- Empfehlungen für sportliche Aktivitäten müssen immer Trainingsform, Reizdauer, Reizumfang und Intensität vorab festgelegt werden (Wagner & Becker, 2008). Die höchste Evidenz und damit den besten Wirksamkeitsnachweis bezüglich der sekundärpräventiven Effekte hat das aerobe Ausdauertraining. Diese Trainingsform wirkt nachweislich auf die kardiovaskuläre Leistungsfähigkeit und trägt wesentlich zur Minimierung der Risikofaktoren einer koronaren Herzkrankheit bei (Hofmann et al., 2009). Jedoch belegen immer mehr Untersuchungen den positiven Effekt eines dynamischen Krafttrainings bei kardialen Patienten (Wonisch et al., 2009). Das aerobe Ausdauertraining und das dynamische Krafttraining ist ein unbestrittener Standard innerhalb der kardiologischen Rehabilitation (Schwan, 2010). Da Übungen zur Verbesserung der Beweglichkeit und Koordination im Vergleich zum aeroben Ausdauertraining und dynamischen Krafttraining keine nachgewiesenen kardioprotektiven Wirkungen haben (Bjarnason-Wehrens et al., 2009), konzentrieren sich die Empfehlungen in dieser Arbeit auf das aerobe Ausdauertraining und das dynamische Krafttraining. Im Folgenden werden die einzelnen Aspekte und Empfehlungen in Bezug auf die KHK und die Belastungsparameter Trainingsform, Trainingsintensität, Trainingsumfang sowie Trainingsdauer konkretisiert.
Traininasform
Aerobes Ausdauertraining: Mögliche Trainingsformen zur Verbesserung der Herzleistung und der kardiovaskulären Leistungsfähigkeit sind Gehen, Nordic Walking, Joggen, Radfahren, Fahrradergometertraining, Schwimmen, Inline-Skaten, Ski-Langlauf, Rudern und Ausdauertraining an Kardiogeräten wiez.B. Stepper, Crosstrainer usw. Am besten untersucht sind die Wirkungen von Laufen und Ergometertraining, da hier die exakte Dosierbarkeit der Belastungsintensität bei gleichzeitigem Monitoring mittels EKG und Blutdruck durchführbar ist (Bjarnason-Wehrens et al., 2009, S. 13). Jedoch stellen alle genannten Trainingsformen eine gute Möglichkeit für den KHK-Patienten dar, sich entsprechend körperlich zu belasten. Bei der Wahl der Trainingsform sollten die persönlichen Voraussetzungen des Patienten berücksichtigt werden. Hierzu zählen das Alter, die momentane Belastbarkeit und Leistungsfähigkeit sowie zusätzliche Komorbiditäten. Bei adipösen Personen sollten bevorzugt Sportarten zur Anwendung kommen, die eine Entlastung des Körpergewichts ermöglichen, Z.B. Radfahren und Schwimmen. Im Sinne des langfristigen, nachhaltigen sportlichen Aktivseins sollen die persönlichen Neigungen und Interessen der Person eine starke Berücksichtigung bei der Empfehlung von bestimmten Trainingsformen erhalten (Bjarnason.Wehrens et al., 2009, S. 14). Dynamisches Krafttraining: Mögliche Trainingsformen sind Übungen an sogenannten Sequenztrainingsgeräten oder auch Krafttrainingsgeräten. Diese Trainingsform bietet eine gute exakte Dosierungsmöglichkeit und eine vorgegebene Führung der Bewegungsbahn und ist für motorisch, koordinativ ungeübte Personen geeignet und zu empfehlen (Schwan, 2010, S. 114). Neben den Krafttrainingsgeräten sind Widerstandsübungen ebenfalls mit kleinen Hilfsmitteln wie Therabänder, kleine Hanteln oder nur mit dem eigenen Körpergewicht in Form von gymnastischen Übungen möglich. Diese Übungen sind schwieriger dosierbar und stellen eine höhere Anforderung an die motorisch-koordinativen Fähigkeiten einer Person dar. Vorteilhaft jedoch ist die gute Umsetzbarkeit dieser Trainingsform in den Alltag und in ein entsprechendes Heimtrainingsprogramm (Bjarnason-Wehrens et al., 2009, S.17).
Trainingsintensität:
Aerobes Ausdauertraining: Für die Steuerung der Trainingsintensität bei KHK ist eine Diagnostik zur Leistungsbeurteilung (vgl. Kapitel 2.2.4 Risiko-Nutzen Abwägung) erforderlich. Aus den gewonnenen Ergebnissen der Spiroergometrie und Laktatdiagnostik können wichtige Parameter zur Trainingssteuerung gewonnen werden. Wichtige Parameter in diesem Kontext sind: die maximale Sauerstoffaufnahme (V02max), die Sauerstoffaufnahme an der aeroben und anaeroben Schwelle, die maximale Herzfrequenz (HFmax), die ermittelte Herzfrequenzreserve (HFR) und Laktaktwerte an der aeroben und anaeroben Schwelle (Bjarnason-Wehrens et al., 2009, S.15). Neben diesen objektiven Parametern wird mit Hilfe der Borg-Skala das subjektive Anstrengungsempfinden der Person erhoben und mit den aus der Diagnostik abgeleiteten Zielwerten abgeglichen (Borg et al., 2010). Bei den Empfehlungen zur Trainingsintensität ist es hilfreich, sich an der Herzfrequenz und dem persönlichen Belastungsempfinden des Patienten zu orientieren. In den meisten Studien sind Intensitäten nach der Dauermethode im Bereich von 40-80% der V02max untersucht worden. Hierbei konnte die kardiovaskuläre Leistungsfähigkeit der Herzpatienten je nach Untersuchung zwischen 11% und 36% verbessert werden (Haskell et al., 2007 ; Balady étal., 2007 ; Leon et al., 2005). Die aktuellen Leitlinien der führenden nationalen und internationalen kardiologischen Fachgesellschaften empfehlen für KHK-Patienten Herzfrequenzen im mittleren Belastungsniveau (Niederseer& Niebauer, 2010, S.171). Konkret bedeutet dies 6075% der maximalen Herzfrequenz oder 40-60% der Herzfrequenzreserve. Empfehlungen mittels spiroergometrischer Untersuchung sind im aeroben Stoffwechsel bei 40-80% der maximalen Sauerstoffaufnahmekapazität angesiedelt (Bjarnason-Behrenset al., 2009, S.15). Die Herzfrequenzempfehlung sollte sich immer an der ischämie- und beschwerdefreien Kapazität des Patienten orientieren. Dynamisches Krafttraining: Das Krafttraining hat auf Grund der kurzen Belastungszeiten und dem lokalen Einsatz der Muskulatur nur einen sehr geringen Einfluss auf die Herzfrequenz. Zur Steuerung der Intensität ist die Herzfrequenz als alleiniges Kriterium im Vergleich zum Ausdauertraining ungeeignet (Schwan, 2010, S.114). Bei Kraftbelastungen gibt die erreichte Herzfrequenz wenig Auskunft über die tatsächliche Belastung (Bjarnason-Wehrens, 2004, S.366). Auf Grund des Blutdruckanstieges bei Kraftbelastungen und dem damit verbundenem kardialen Risiko war man in der Vergangenheit mit therapeutischen Interventionen mittels Krafttrainingsmethoden eher zurückhaltend. Intraarteriell durchgeführte Messungen des Blutdruckes bei Intensitäten zwischen 40%-60% des 1-RM haben jedoch nur einen geringen Anstieg des Blutdruckes zur Folge ( Pollocketal., 2000). Die Deutsche Gesellschaft für Prävention und Rehabilitation von Herz-Kreislauferkrankungen (DGPR) empfehlen für KHK-Patienten ein dynamisches Krafttraining (Bjarnason-Wehrens et al., 2009). Intensitäten im Krafttraining werden üblicherweise im Krafttraining Überdas Einwiederholungsmaximum (1-RM) objektiviert und festgelegt. Zur Ermittlung der Trainingsintensität ist das 1-RM Verfahren jedoch aus zweierlei Hinsicht für KHK- Patienten nicht empfehlenswert. Erstens ist ein solches Vorgehen bei koronaren Herzpatienten auf Grund von auftretendem Pressdruck und den damit verbundenen Blutdruckspitzen risikobehaftet und zweitens besteht zwischen dem 1-RM und der abgeleiteten prozentualen Intensität sowie der damit verbundenen Wiederholungszahl kein signifikanter Zusammenhang (Schwan, 2010, S. 14). Folgendes Vorgehen zur Bestimmung der Trainingsintensität hat sich in der praktischen Arbeit mit Herzpatienten etabliert: Die Austestung erfolgt über das subjektive Belastungsempfinden der Person an Hand der Borg-Skala durch stufenweise Steigerung des Widerstandes bei einer bestimmten vorgegebenen Wiederholungszahl (Bjarnason- Wehrens, 2004, S.366) Hierbei ist insbesondere auf eine Vermeidung der Pressatmung zu achten. Um Überlastungen und kritische kardiale Situationen zu vermeiden, wird der Patient stufenweise langsam an eine entsprechende Belastung herangeführt. Die entscheidenden Belastungsgrößen im Krafttraining sind die Wiederholungszahlen und Kraftintensitäten. Die Intensitäten und Übungen sollten so gewählt werden, dass die Person die gewünschte Wiederholungszahl ohne Pressatmung bei korrekter Übungsausführung und beschwerdefrei durchführen kann. Empfohlen werden Intensitäten zwischen 30% - 60% vom 1-RM bzw. des subjektiven Belastungsempfindens (Bjarnason-Wehrens et al., 2009).
Traininasdauer und -umfang
Aerobes Ausdauertraining: In einer älteren Heidelberger Regression Studie zeigte sich bei einem Energieverbrauch von 2200 kcal/pro Woche durch aerobes Ausdauertraining eine Senkung des Stenosegrades bei den Koronargefäßen (Schuler et al., 1992). Ein kalorischer Umsatz in der Größenordnung entspricht in etwa 5-6 stunden gezielter Bewegungsaktivität. Die „Leitlinie körperliche Aktivität zur Sekundärprävention und Therapie kardiovaskulärer Erkrankungen“ ist eine Stellungnahme der Deutschen Gesellschaft für Prävention und Rehabilitation von Herzkreislauferkrankungen (DGPR), die den gegenwärtigen Erkenntnisstand wiedergibt und Ärzten, Bewegungs- und Sporttherapeuten die Entscheidungsfindung und Beratung bezüglich körperlicher Aktivität und Training erleichtern soll (Bjarnason-Wehrens et al., 2009). Nach diesen Empfehlungen sollen KHK- Patienten ein aerobes Ausdauertraining mit einem Umfang von 3-7 Einheiten an möglichst vielen Tagen mit einem Gesamtumfang von 150 - 180 Minuten pro Woche durchführen. Optimale Effekte bezüglich der kardiprotektiven Wirkung und Verbesserung der Leistungsfähigkeit sind bei einer Dauer von mindestens 20 Minuten, besser 30-45 Minuten zu erwarten (Niederseer & Niebauer, 2010). Diese Empfehlungen stimmen mit den Angaben der Deutschen Gesellschaft für Kardiologie und der European Society of Cardiology sowie der American Heart Association überein (Niederseer & Niebauer, 2010) Die Hinweise aller Fachgesellschaften stützen sich auf entsprechende Studien und Untersuchungen, bei denen der sekundärpräventive Effekt durch die entsprechende Trainingshäufigkeit nachgewiesen wurde (Haskell et al., 2007 ; Balady et al., 2007). Dynamisches Krafttraining: Die empfohlene Trainingshäufigkeit liegt laut der Deutschen Gesellschaft für Prävention und Rehabilitation von Herz-Kreislauferkrankungen bei 2- 3 Trainingseinheiten pro Woche und bei 1- 3 Serien pro Muskelgruppe (Bjarnason-Wehrens et al., 2009).
Die Empfehlungen der verschiedenen Belastungsparameter für ein Ausdauer- und Krafttraining für KHK-Patienten sind noch mal übersichtlich in der folgenden Tabelle aufgeführt.

Die hohen Dropout-Quoten von bis zu 60% bei Gesundheitsprogrammen deuten daraufhin, dass die Empfehlungen der Fachgesellschaften als erster Einstieg besonders für inaktive Patienten eine zu große Hürde darstellt (Löllgen, 2003). Es gibt auch Belege dafür, dass mehrere kurze Belastungen von 10 Minuten am Tag in Form von Alltagsaktivitäten insbesondere auf die Endothelfunktion einen positiven Einfluss haben (Hambrecht et al., 2000). Jede körperliche Aktivität in Form einer Belastung, die über das normale Alltagsniveau hinausgeht und den kalorischen Umsatz erhöht, hat einen günstigen Effekt auf die KHK (Löllgen, 2013). Die Dosis-Wirkungsbeziehung zwischen Trainingsumfang und Mortalität ist nicht linear. Besonders inaktive Personen haben bezüglich der Sterblichkeit relativ gesehen den höchsten Nutzen, wenn Sie mit einem regelmäßigen und moderaten Training beginnen (Löllgen, 2013). Die entscheidende Risikominimierung liegt beim Übergang von der „ Inaktivität“ zu einem moderaten Training. Zwei bis zweieinhalb stunden sportliche Aktivität
pro Woche ist die entscheidende Größe oberhalb derer eine nachhaltige Wirkung einritt (Löllgen, 2013). Ein Mehr an sportlicher Aktivität verbessert zwar die Leistungsfähigkeit, das relative Sterblichkeitsrisiko verändert sich jedoch unwesentlich. Der Zusammenhang von keiner, moderater und intensiver sportlicher Aktivität und das relative Sterblichkeitsrisiko ist in der nachfolgenden Abbildung dargestellt.

Körperliches Training ist für KHK Patienten mit einem erhöhten relativem Risiko verbunden (Albert et al., 2000). Dieses relative Risikopotential ist jedoch bei der Betrachtung des absoluten Risikos zu vernachlässigen und darf nicht überbewertet werden. Das absolute Risiko von kardiovaskulären Ereignissen unter Trainingsmaßnahmen ist sehr gering. Bei Personen mit entsprechender vordiagnostizierter KHK liegt das Risiko eines tödlichen Ereignisses bei 1 zu 116.906 Trainingsstunden. Bezogen auf die Patienten liegt das Risiko bei 1 zu 81.670 Patienten (Thompson et al., 2007). Die Zahlen verdeutlichen das sehr geringe Risiko, einem plötzlichen Herztod unter sportlicher Aktivität zu erliegen. Der Nutzen von körperlicher Bewegung für einen KHK-Patienten kann als deutlich größer eingestuft werden als das Risiko eines kardiovaskulären Ereignisses in Form eines plötzlichen Herztodes. Ganz im Gegenteil, es gibt zahlreiche Belege dafür, dass gerade körperliches Training die sportbedingten und allgemeinbedingten kardiovaskulären Zwischenfälle reduziert (Thompson et al., 2007). Das erhöhte relative Risiko und die Unsicherheit der Patienten bezüglich ihrer körperlichen Belastbarkeit erfordern jedoch spezielle, insbesondere diagnostische Strategien um bewegungsassoziierte kardiale Ereignisse zu vermeiden (Niederseer & Niebauer, 2010, S. 173). Neben einer gründlichen Anamnese ist eine
Eingangs- und Basisdiagnostik sowie eine weiterführende Diagnostik zur Leistungsbeurteilung und Risikoevaluation bei KHK-Patienten erforderlich (Bjarnason-Wehrens et al., 2009). Das Flussschema der Abbildung vier beschreibt anschaulich das diagnostische Vorgehen zur Risikostratefizierung der KHK-Patienten hinsichtlich der Empfehlungen zur körperlichen Aktivität.

In Deutschland gibt es ein sehr umfassendes System der kardiologischen Rehabilitation, das dem KHK-Patienten eine langfristige und erfolgreiche Versorgung sichert (Bjarnason- Wehrens et al., 2006, S.559). Nach der Versorgung im akutmedizinischen Bereich (Phase I) hat der Patient einen gesetzlichen Anspruch auf eine stationäre oder ambulante Anschlussheilbehandlung (Phase II). Die Phase III der kardiologischen Rehabilitation umfasst verschiedene Nachsorgeangebote (Haberecht, 2010, S.226). In Deutschland können KHK- Patienten nach einer Akutbehandlung im Krankenhaus an einer stationären oder ambulanten Anschlussheilbehandlung (AHB) in einer Rehabilitationseinrichtung teilnehmen und sich umfassend versorgen lassen. Für den KHK- Patienten besteht in aller Regel eine Reha- Bedürftigkeit nach überstandenem akuten Koronarsyndrom. Diese Bedürftigkeit ist insbesondere dann gegeben, wenn durch die Erkrankung eine Beeinträchtigung der körperlichen, seelischen und sozialen Integrität gegeben und damit die Teilhabe in den beruflichen und sozialen Lebensbereichen gefährdet ist (Korsukewitz et al., 2007, S.11).
Nach der Versorgung im Akutkrankenhaus kommt der Patient, sobald er rehabilitationsfähig ist in eine Rehabilitationseinrichtung, übergeordnetes Rehabilitationsziel und gesetzlicher Auftrag der Rehabilitation ist es, die Wiedererlangung der Selbstständigkeit und Autonomie, die bestmögliche physische und psychische Gesundheit sowie die soziale und berufliche Integration des Patienten wiederherzustellen (Korsukewitz et al., 2007, S. 11). Eine wesentliche Aufgabe der Rehabilitation ist es die betroffenen KHK- Patienten bezüglich ihres individuellen Risikoprofils aufzuklären, zu schulen und auf eine Lebensstilveränderung hinzuwirken (Karoff et al., 2010). Hierbei spielt die Hinführung der Rehabilitanden zu einem körperlich aktiven Lebensstil eine bedeutende Rolle (Geidl et al., 2012). Die prognostisch günstigen Effekte bei einer bestehenden koronaren Herzerkrankung durch körperliche Aktivität im Sinne der Sekundärprävention sind wissenschaftlich umfangreich untersucht und nachgewiesen. In mehreren ausreichend großen, randomisierten Studien oder Metaanalysen konnte die Wirksamkeit belegt werden (Bjarnason-Wehrens, 2009). Eine ausführliche Darstellung hierzu wurde bereits im Kapitel 2.2 dieser Arbeit gegeben. Körperliches Training wirkt sich als alleinige Maßnahme positiv auf mehrere Risikofaktoren der koronaren Herzerkrankung aus und ist ein wichtiger Baustein in jeder stationären und ambulanten kardiologischen Rehabilitationsmaßnahme (Müller-Fahrnowetal., 2006). Eine Erhebung von Brügemann & Sewöster (2010) zeigt, dass der Anteil bewegungstherapeutischer Maßnahmen in der kardiologischen Rehabilitation im Durchschnitt 9,4 stunden pro Woche beträgt. Damit sind die bewegungstherapeutischen Interventionen die umfangsstärksten Therapieleistungen im Therapieplan des KHK-Patienten (Brügemann & Sewöster, 2010). Hierdurch wird die Bedeutung der bewegungstherapeutischen Maßnahmen im Kontext der Rehabilitation bei KHK-Patienten deutlich. Der kurzfristige Effektivitätsnachweis der stationären kardiologischen Rehabilitation auf wichtige Parameter und Risikofaktoren im Zusammenhang mit der KHK wie körperliche Leistungsfähigkeit, systolischer und diastolischer Blutdruck, Lipidstatus und Körpergewicht konnte in mehreren Untersuchungen gut nachgewiesen werden ( Küpper- Nybelen et al., 2003 ; Völler et al., 2000 ; Müller et al., 2009). Eine kontinuierliche Fortführung des körperlichen Trainingsprogramms nach dem stationären Aufenthalt im Sinne der Therapietreue und Adhärenz hätte demnach für die Patienten einen hohen gesundheitlichen sekundärpräventiven Nutzen. Der Erfolg der Rehabilitation hängt wesentlich davon ab, dass der KHK- Patient im Anschluss an den stationären Aufenthalt das in der Klinik initiierte körperliche Training selbstständig an seinem Wohnort weiterführt, Z.B. im Rahmen einer ambulanten Herzsportgruppe oder durch selbstbestimmte sportliche Aktivitäten (Buchwalsky et al., 2002). Für den langfristigen Rehaerfolg ist es entscheidend, dass gerade Herzpatienten nach Abschluss der stationären Behandlung die medizinisch indizierten Verhaltensweisen kontinuierlich weiterführen (Bengel & Herwig, 2003; Buchalsky & Held, 2002). Ohne eine entsprechende Lebensstiländerung ist es oft nur eine Frage der Zeit, bis sich die körperlichen und psychischen Symptome wieder so verschlechtern, dass eine erneute Behandlungs- bzw. Rehabilitationsbedürftigkeit entsteht. Die Wichtigkeit der Nachsorge (Reha - Phase III) wurde auch von den Leistungsträgern der medizinischen Rehabilitation erkannt. Um die kurzfristigen Rehabilitationserfolge auch nachhaltig durch eine entsprechende Lebensstilmodifikation zu sichern, wurden unterschiedliche Nachsorgeprogramme konzipiert mit dem Ziel, die Anfangserfolge und Ergebnisse der stationären oder ambulanten Rehabilitation dauerhaft und nachhaltig zu sichern. Im Rahmen einer ergänzenden Leistung der gesetzlichen Krankenkassen hat der Patient zum Beispiel die gesetzlich gesicherte Möglichkeit, an einer der ca. 6500 flächendeckenden und wohnortnah organisierten Herzsportgruppen teilzunehmen (Held, 2007, S.7). Der von den gesetzlichen Krankennkassen garantierte und für den Patienten kostenfreie Leistungsumfang beträgt 90 Übungseinheiten, die innerhalb von 24 Monaten in Anspruch genommen werden müssen (Haberecht, 2010, S.447). Weiterhin bietet die Deutsche Rentenversicherung die Nachsorgeprogramme KARENA (Kardiovaskuläres Rehanachsorgeprogramm), IRENA (Intensivierte Rehabilitationsnachsorge), ARENA (Ambulante Rehabilitationsnachsorge) und INA (Intensivierte Nachsorge) an. Diese Programme können von den Versicherten der Deutschen Rentenversicherung nach einer stationären oder ambulanten Rehabilitationsmaßnahme in Anspruch genommen werden und müssen von dem jeweiligen Rehamedizinern beantragt und für den Patienten eingeleitet werden. Obwohl gerade diese Patienten sehr stark von aktiven Bewegungsmaßnahmen profitieren könnten, sind bei diesen Patienten der inaktive Lebensstil und fehlende Therapietreue ein sehr typisches Charakteristikum (Geidl et al., 2012). Trotz dieser intensiven Bemühungen gelingt es nur wenigen Patienten im Anschluss an eine Rehabilitation, die neu erlernten Bewegungsaktivitäten in ihren Alltag zu integrieren (Wagner, 2007, S.71-88). Die Befunde aus verschiedenen Studien zeigen, dass es offenbar nur einen geringen Prozentsatz der Rehabilitanden gelingt, ein nachhaltiges, stabiles Aktivitätsverhalten in eigener Verantwortung aufzubauen (Marzolini et al., 2010). Studien der Deutschen Rentenversicherung berichten über eine wiederholte Inanspruchnahme von Rehaleistungen innerhalb eines Fünf-JahresZeitraums von 20% (Klosterhuis, Zollmann & Grünbeck, 2004). Wie schwierig es ist, mit einem regelmäßigen Sport- und Bewegungsprogramm zu beginnen und dieses kontinuierlich aufrechtzuerhalten, ist an den Teilnahmezahlen der kardiologischen Patienten an einer Ambulanten Herzsportgruppe zu erkennen. Nur ein geringer Teil 20-30% der poststationären Patienten schließen sich solch einer Herzsportgruppe an (Schlicht, Kanning & Bös, 2003), andere Autoren erwähnen größere Spannbreiten und Teilnahmequoten von 13-40% (Bjarnason-Wehrens, 2006). Erschwerend kommt hinzu, dass ca. 30% der Teilnehmer die
Herzsportgruppe nach 6-7 Monaten wieder verlassen (Schlicht, Kanning & Bös, 2003). Im Rahmen von verschiedenen prospektiven Kohortenstudien wurde untersucht, inwieweit die kurzfristigen Rehabilitationserfolge dauerhaft und nachhaltige Effekte bei den Patienten erzielen konnten. Die sekundärpräventiven Ergebnisse im Hinblick auf eine nachhaltige Verhaltensänderung sind eher enttäuschend ( Küpper-Nybelen et al., 2003 ; Völler et al., 2000). Der positve Einfluss auf die lebensstilbedingten Risikofaktoren der KHK durch die stationäre Rehabilitation verliert sich und ist eher rückläufig. Die EUROASPIRE I, II und III- Untersuchungen in der Region Münster zeigen eindrücklich, wie sich ein zeitlicher Trend von kardiovaskulären Risikofaktoren bei Patienten mit KHK über einen Zeitraum von mehr als einem Jahrzehnt auswirkt. Die Zunahme einzelner kardiovaskulärer Risikofaktoren nach einer entsprechenden medizinischen Behandlung waren in den klinischen Untersuchungen zu allen drei Erhebungszeitpunkten (1995/96; 1999/2000 und 2006/2007) sehr gut nachzuweisen (Prugger et al., 2012). Anscheinend sind ein Herzinfarkt, ein Bypass oder ein Stent für viele Patienten mit koronarer Herzkrankheit kein ausreichendes Motiv, ihren Lebensstil zu ändern. Diese alleinige Erklärung wäre jedoch zu einfach und würde der Thematik der Verhaltensmodifikation wissenschaftlich nicht gerecht. In der Motivations- und Volitionsforschung hat sich mittlerweile der Ansatz durchgesetzt, dass eine Verhaltensänderung nicht allein von der Absicht oder auch dem Ausmaß einer der Motivation abhängig ist. Bei der theoretischen Aufarbeitung hat sich die Unterscheidung einer motivationalen und einer volitionalen Phase etabliert. Im Gegensatz zu den bisher dominierenden funktionellen und trainingstherapeutischen Ansätzen haben verhaltensbezogene Interventionen ein gutes Potential, das Bewegungsverhalten nachhaltig zu verändern und zu beeinflussen. Die Wirksamkeit und damit die Evidenz von gezielten Interventionstechniken zur dauerhaften Veränderung des Bewegungsverhaltens im Rehabilitationssetting konnte in verschiedenen Kontrollgruppenstudien nachgewiesen werden (vgl. Kap.2.5). Insbesondere volitionsorientierte Interventionen zeigen hier gute
Effektivitätsnachweise. Professionelle und adhärenzfördernde Interventionsangebote für die Patienten von Seiten der Rehabilitationskliniken sind eine gute Möglichkeit, die Patienten bei einem dauerhaften Aufbau eines körperlich aktiven Lebensstils zu unterstützen. Das theoriegestützte und ทาodualisierte Interventionsprogramm MoVo-LISA von der Freiburger Forschergruppe um Reinhard Fuchs ist eine mögliche Option und soll in dieser Arbeit auf seine Wirksamkeit bei stationären KHK-Patienten überprüft und evaluiert werden.
Seine Gewohnheiten zu ändern und eine Verhaltensveränderung herbeizuführen, ist eine schwierige Aufgabe (Sniehotta et al., 2007, S.150). Regelmäßiges Sporttreiben ist eine wichtige Komponente für einen KHK-Patienten und gilt als ein protektiver Faktor zur Vermeidung eines erneuten kardialen Ereignisses (vgl. Kap. 2.2.2). Folglich ist es von entsprechend großer Bedeutung, die psychologischen Einflussfaktoren und Modelle zu identifizieren, die die Umsetzung von Intentionen in regelmäßiges Sporttreiben fördern. Im folgenden Abschnitt dieser Arbeit werden unterschiedliche theoretische Modelle und Faktoren vorgestellt, die in den letzten Jahren zur Erklärung von Gesundheitsverhaltensänderungen im Bereich der körperlichen Aktivität verwendet wurden. Dabei wird zunächst der Begriff der Motivation geklärt und an Hand der Selbstbestimmungstheorie nach Ryan & Deci (2000) weiter spezifiziert. Anschließend wird der Unterschied zwischen Motivation und Volition an dem Rubikonmodell erläutert. Danach werden die verschiedenen Erklärungs- und Interventionstheorien übersichtlich dargestellt. Beispielhaft werden hierzu zwei bedeutsame Vertreter, die Sozial-kognitive Theorie von Bandura (2000) und das Transtheoretische Modell von Prochaskaund DiClemente (1992), näher beschrieben. Darauffolgend wird das häufig in der Literatur erwähnte Phänomen der Intentions-Verhaltenslücke erklärt. Nach diesem allgemein theoretischen überbau wird das für die MoVo-LISA-lntervention bedeutsame MoVo-Prozessmodell von Fuchs (2007, S.317-325) indem Kontext der Verhaltensänderung vorgestellt. Aufbauend auf dem Verständnis des MoVo-Prozess-Modells wird anschließend das in der RCT-Studie verwendete Interventionsprogramm MoVo-LISA erläutert.
Der Begriff Motivation wird umgangssprachlich sehr oft benutzt. Häufig bezeichnen wir Personen, die sich für eine Sache sehr begeistern können, als motiviert. Ein kardiologischer Rehabilitand, der innerhalb der Rehabilitation an allen Therapien sehr interessiert ist, neben den Standardtherapien noch zusätzlich sportlich aktiv ist und auf seine Ernährung achtet, wird von den zuständigen Ärzten und Therapeuten in den Teamsitzungen als sehr motiviert eingestuft. In der Wissenschaft hat die Motivationsforschung eine lange Tradition und es gibt zahlreiche Definitionen zu dem Begriff Motivation. Heckhausen (2006) beschreibt im Vorwort seines Buches zu dem Thema „Motivation und Handeln“ die Komplexität der Thematik wie folgt: „ Kaum ein Gebiet der psychologischen Forschung ist von so vielen Seiten zugänglich wie die Motivationspsychologie und doch zugleich so schwer zu überschauen.“ Rheinberg hat zum Thema Motivation folgende drei Verhaltens - und Erlebnismerkmale herausgearbeitet.
Motiviert sein im Sinne von Rheinberg bedeutet: 1. jemand hat ein Ziel, 2. er strengt sich an und 3. er bleibt ablenkungsfrei bei der Sache (Rheinberg, 2008, S.14). Bei der Begriffsklärung liefert Rheinberg eine übergreifende Definition. Erbezeichnet Motivation als eine „aktivierende Ausrichtung des momentanen Lebensvollzugs auf einen positiv bewerteten Zielzustand“ (Rheinberg, 2008, S. 16). In der Literatur wird zwischen extrinsischer und intrinsischer Motivation unterschieden (Rheinberg, 2006). Zur Unterscheidung dieser beiden Formen ist nicht das Verhalten als solches von Bedeutung, sondern die Erwartung oder auch Konsequenz des Verhaltens, die eine Person an ihre Handlung hat. Hierbei kann ein und dieselbe Handlung einmal extrinsischer und ein anderes Mal intrinsischer Natur sein. So kann beispielsweise eine Person zum Ausdauersport motiviert sein, um Gewicht zu reduzieren oder um einer koronaren Herzerkrankung vorzubeugen. In diesem Beispiel betreibt die Person den Sport, um ein bestimmtes Ziel zu erreichen. Hier ist die Erwartung an das Ergebnis der Handlung gebunden, welches die eigentliche Triebfeder des Handelns ist. Solch ein Verhalten wird in der Wissenschaft als extrinsische Motivation bezeichnet (Scholz et al, 2007, S. 132). Eine andere Person betreibt den Ausdauersport nicht um irgendetwas Bestimmtes zu erreichen, sondern um seiner selbst willen. Die Handlung ist nicht an bestimmte Erwartungen gekoppelt. Solch ein Verhalten wird in der Wissenschaft als intrinsische Motivation bezeichnet (Scholz et al, 2007, S.132). Ryan und Deci (2000) beleuchten in ihrer Selbstbestimmungstheorie den Unterschied zwischen extrinsischer und intrinsischer Motivation aus einer differenzierteren Perspektive. Das Grundbedürfnis des Menschen nach Selbstbestimmung des eigenen Handelns verbunden mit dem Gefühl, selbst die Quelle des eigenen Handelns zu sein, ist die Grundlage der self-determination theory ( Ryan & Deci, 2000). In der Selbstdeterminisierungstheorie beschreiben Ryan & Deci verschiedene Motivationsarten. Eine handelnde Person befindet sich demnach immer auf einem Kontinuum zwischen bestimmten Motivationsmodi. Diese reichen von einem nicht selbstdeterminierten und amotivierten Zustand über vier verschiedene stufen der extrinsischen Motivation (von „ external reguliert“ über „introjiziert reguliert“ und „identifiziert reguliert“ bis hin zu „integriert reguliert“) und enden schließlich in einem selbstdeterminierten intrinsischen Motivationszustand (S. Abb. 5, S.29). Amotivation liegt vor, wenn die Person keinerlei Bezug zu einem bestimmten Verhalten aufweist und somit auch keinerlei Handlungsabsichten besitzt (Ryan & Deci, 2000). Im external regulierten Modus handelt eine Person allein durch positive oder negative Regulation von außen. Im introjizierten Modus handelt die Person schon aus verinnerlichten Gründen, jedoch sind es noch nicht gänzlich die wirklich „eigenen.“ Die Handlungen im introjizierten regulierten Verhalten werden von dem inneren Wertesystem noch nicht zu den eigenen, selbstbestimmten gezählt. Handelt eine Person aus freier Entscheidung für sich selbst und aus persönlicher innerer Überzeugung, so liegt nach (Ryan & Deci, 2000) der identifizierte Modus vor. Das Verhalten im Modus der integrierten Regulation ist gänzlich selbst bestimmt und wird als eins mit dem Selbst erlebt. Die Person führt die Handlung um ihrer selbst willen durch, und ist intrinsisch motiviert (siehe Abb. 5, S.29). In der gelebten Wirklichkeit könnten sich die theoretisch beschriebenen Modi beispielhaft wie folgt bei einem KHK-Patienten darstellen: Vor Eintritt des akuten Koronarsyndroms in Form eines Herzinfarktes hatte die Person keinerlei Bezug zu sportlichen Aktivitäten und nicht einen Gedanken damit verbracht, sportlich aktiv zu werden. Sie war bezogen auf das Sportverhalten nach der Theorie von Ryan & Deci in einem amotivierten Modus. Nach dem Herzinfarkt und der akutstationären Behandlung im Krankenhaus absolviert die Person nun eine stationäre kardiologische Rehabilitationsmaßnahme in einer Rehaklinik. Die Person nimmt lustlos und gelangweilt nur deshalb am Ergometertraining in der Klinik teil, weil der Arzt es verordnet hat und er durch die Teilnahme einen negativen Abschlussbericht des Arztes vermeiden möchte. Dieses Verhalten kennzeichnet den external regulierten Modus. Introjizierter Modus würde vorliegen, wenn der Patient am Ergometertraining teilnimmt, weil sein Arzt ihm die gesundheitlichen Vorteile der sportlichen Aktivität erklärt hat, jedoch ist der Patient noch nicht wirklich davon überzeugt. Ein Verhalten im identifizierten Modus ist gegeben, wenn der Patient von den gesundheitlichen Wirkungen des Ergometertrainings persönlich selbst überzeugt ist und aus freier Entscheidung daran teilnimmt. Ein Patient, der sich gar keine Gedanken darüber macht, warum er jetzt am Ergometertraining teilnehmen soll, da er sich einfach gerne bewegt und mit Freude und Spaß um seiner selbst willen das Training absolviert, befindet sich im intrinsischen Modus. Die beschriebene Selbstbestimmungstheorie mit ihren verschiedenen Abstufungen zur Erklärung der Motivation für körperliche Aktivität bezeichnet Scholz als intuitiv einleuchtend (Scholz et al., 2007, S.146). Das Motivationsverständnis nach Ryan und Deci (2000) ist im Zusammenhang einer dauerhaften und nachhaltigen Verhaltensänderung bedeutsam. Dass Personen nur dann nachhaltig Sport treiben, wenn die Bewegung zu ihnen passt und ihren eigenen Wünschen und Absichten entspricht, zeigt eine Studie mit adipösen Patienten. Die Arbeitsgruppe von Teixera konnte im Kontext der Gewichtsreduktion mit adipösen Patienten das Vorliegen eines integrierten Motivationsmodus als wichtigsten Prädiktor zum dauerhaften sportlichen Aktivsein in dieser Untersuchung herausarbeiten (Teixera et al., 2006). In weiteren Studien konnte gezeigt werden dass, wenn das zukünftig gewünschte Verhalten mit den persönlichen Interessen und Werten einer Person im Einklang stand, es seltener zum Abbruch des zielführenden Verhaltens kam ( Sheldon & Houser- Marko, 2001; Koestner et al., 2002). Selbstkonkordante Ziele mit großer Ich-Nähe lösen bei der Person eine höhere Anstrengungsbereitschaft aus und führen zu einem stärkerem Wohlbefinden (Sheldon, 2002).

In diesem Zusammenhang wird deutlich, dass die persönliche Überzeugung und auch die eigenen Wertvorstellungen und Interessen einer Person bei der Hinführung zu einer Verhaltensänderung von großer Wichtigkeit sind. Motivationsarbeit bedeutet in diesem Zusammenhang, die Person an die Hand zu nehmen und sie so zu unterstützen, dass ihr individueller, sozialer und beruflicher Lebenskontext berücksichtigt wird.
Der Wunsch einer Person, etwas Bestimmtes zu erreichen, garantiert, wie wir alle aus leidvoller Erfahrung wissen, noch nicht, dass wir auch entsprechend handeln. Auch wenn Menschen hoch motiviert sind, in Zukunft sportlich aktiv zu sein, gelingt es Ihnen oft nicht, ihre guten Vorsätze in die Tat umzusetzen ( Fuchs, 2007, S.317). Was ist also noch notwendig, damit Wünsche sich in Handlungen manifestieren? Mit dieser Frage befasst sich das sogenannte Rubikon-Modell, das den Namen jenes schicksalsträchtigen Flusses trägt, den Julius Cäsar am 10. Januar 49 V. Chr. mit den Worten „alea ¡acta est“ („Der Würfel ist gefallen“) überschritt, um damit zum Ausdruck zu bringen, dass er sich entschieden hatte, gegen Rom zu marschieren. Seinen Wunsch nach mehr Macht hatte er mit dem Überschreiten des Rubikon in eine klare Absicht (Intention) umgesetzt, die für sein weiteres Handeln bestimmend war. Genau darum geht es auch im psychologischen Rubikon-Modell, das die beiden deutschen Psychologen Heinz Heckhausen und Peter Gollwitzer formuliert haben (Gollwitzer, 1990, Heckhausen 1987). Ausgangspunkt für die Entwicklung dieses Modells war es, die zwei Grundprobleme der Motivationspsychologie, nämlich die Wahl von Handlungszielen als Ausdruck des Wollens einerseits und die Realisierung dieser Ziele als Ausdruck des Handelns andererseits zu unterscheiden und in ein gemeinsames Rahmenmodell zu integrieren (Achtziger & Gollwitzer, 2010, S.309 f.). Das Rubikonmodell in der folgenden Abbildung fünf unterteilt den Handlungsverlauf in vier natürliche, nacheinander ablaufende Phasen, die im Folgenden beschrieben werden.

In der prädezisionalen Handlungsphase vor dem überschreiten des Rubikon geht es um die Auswahl von Wünschen und deren Umwandlung in Absichten (Intentionen). In dieser Phase wird auch die Realisierbarkeit oder Machbarkeit des Wunsches geprüft und abgewogen. Nach erfolgreichem Abwägen kommt es zur Bildung einer Zielintention. Mit der Umwandlung von Wünschen in ein klares Ziel entsteht in der Person ein Gefühl des Entschlossenseins; sie ist sich sicher, ihr beschlossenes Ziel verbindlich anzustreben (Achtziger & Gollwitzer, 2010, S.309 f). Mit der Umwandlung eines Wunsches in eine klare Handlungsabsicht und ein Ziel ist bildlich der Rubikon überschritten. In der Person entsteht das Gefühl, das Ziel wirklich in die Tat umzusetzen. Diese Verpflichtung wird in der Forschung auch „Commitment“ genannt (Achtziger & Gollwitzer, 2010, S.309 f). Diesen Prozess bezeichnet man auch als „motivationale Phase“, da Heckhausen den Terminus der Motivation für alle Abläufe benutzt, welche sich mit dem Setzen von Zielen (goal setting) befassen (Achtziger & Gollwitzer, 2010, S.309 f).
Präaktionale Phase des Planens
Mit einer klaren Zielintention und dem überschreiten des Rubikons ist das motivationale Abwägen beendet. Ab sofort steht die Realisierung des angestrebten Ziels im Fokus der Aufmerksamkeit. In der präaktionalen Phase überlegt sich eine Person, mit welcher Strategie und mit welchem Plan er sein Ziel erreichen kann. Die Phase wird auch als „volitionale Phase“ bezeichnet, da die volitionalen Handlungsprozesse in dieser Situation dominieren. Heckhausen beruft sich hier erneut auf die alten Untersuchungen von Kurt Lewin (1926), die mit Volition diejenigen Formen der Motivationsphänomene bezeichnen, die sich ausschließlich auf das streben nach Zielen (goal striving) und mit dem Erreichen von Zielen befassen (Achtziger & Gollwitzer, 2010, S.309 f).
A kt io na le Phase des Handelns
In dieser Handlungsphase versucht die Person die geplanten zielfördernden Maßnahmen tatsächlich durchzuführen und zu realisieren. Wichtig ist hier die beharrliche Verfolgung des angestrebten Zieles. Die mentale Präsenz des Ziels erhöht die Anstrengungsbereitschaft und hilft dabei, Hindernisse zu überwinden (Achtziger & Gollwitzer, 2010, S.309 f).
Postaktionale Phase des Bewertens
In dieser Phase bewertet die Person ihr erreichtes Ergebnis. Ist sie mit dem, was sie durch ihr Handeln erreicht hat, sehr zufrieden, so kann sie das vorher festgelegte Ziel löschen oder, um in den Begrifflichkeiten des Modells zu bleiben, deaktivieren. Ist das Ergebnis nicht erreicht, hat die Person nach Heckhausen zwei Möglichkeiten zu reagieren. Erstens: sie gibt sich mit dem bisher Erreichten zufrieden und korrigiert somit ihr Anspruchsdenken, in diesem Fall deaktiviert sie ebenfalls das vorher festgelegte Ziel. Eine weitere Reaktionsmöglichkeit wäre, das Ziel nicht zu löschen, sondern neue Strategien und Handlungspläne zu schmieden, die mehr Aussicht auf Erfolg gewährleisten können (Achtziger & Gollwitzer, 2010, S.309 f).
Fazit
Das Rubikonmodell differenziert vier verschiedene Phasen des Handelns und erklärt diese an Hand der Begriffe Abwägen, Planen, Handeln und Bewerten. In jeder einzelnen Phase befindet sich die Person in einer anderen Bewusstseinslage. Eine zentrale Erkenntnis des Rubikon-Modells ist die Tatsache, dass sich die gedanklichen Inhalte, die Steuerung der Aufmerksamkeit und die motivationale Bewusstseinslage in den einzelnen Phasen unterscheiden (Achtziger & Gollwitzer, 2010, S.309 f). Motivationale Prozesse dominieren vor allem in der prädezisionalen Phase des Abwägens und unter Umständen in der postaktionalen Phase des Bewertens. Volitionale Handlungsmerkmale, die mit der konkreten Realisierung von Zielen im Handeln zu tun haben, finden in der präaktionalen Phase des Planens und in der aktionalen Phase des Handelns statt.
Inaktive sportliche Personen zu einem dauerhaften sportlichen Lebensstil hinzuführen, ist eine schwierige Aufgabe. Vor diesem Hintergrund sollten die Konzeptionen von sportfördernden Programmen eine starke theoretische Fundierung haben, um die Wirksamkeit dieser Maßnahmen zu verbessern (Fuchs, 2003, S. 115). Nur wenn man die Mechanismen der Verhaltensänderung versteht, kann man eine maßgeschneiderte Intervention für die entsprechende Zielgruppe entwickeln. Fuchs (2003, S.109) unterscheidet bei den theoretischen Grundlagen zu sportbezogenen Interventionen zwischen Erklärungstheorien und Interventionstheorien. Innerhalb der Erklärungstheorien wird zum besseren Verständnis weiterhin zwischen strukturmodellen und Stadienmodellen unterschieden. Erklärungstheorien liefern das Wissen und Verständnis über die Bedingungen des zu verändernden Verhaltens. Dieses Wissen ist vor allem für die Planung einer Intervention entscheidend, da entsprechende Faktoren identifiziert werden können, die für das Initiieren und Aufrechterhalten von sportlichen Aktivitäten von besonderer Bedeutung sind (Fuchs, 2003, S.121). Strukturmodelle als ein Teil der Erklärungstheorien gehen davon aus, dass die Wahrscheinlichkeit einer Veränderung des Verhaltens durch bestimmte Einflussgrößen, wie zum Beispiel einer hohen Zielintention, verstärkt werden kann. Lippke& Kaluschke erklären diesen Ansatz mit dem Bild einer mathematischen Gleichung. Erhöht man auf der einen Seite der Gleichung den Wert, in diesem Kontext sind damit bestimmte Einflussgrößen des Verhaltens, die sogenannten Prädiktorvariablen gemeint, so wird auch das Ergebnis auf der anderen Seite größer. Damit ist die angestrebte Verhaltensänderung gemeint (Lippke & Kalusche, 2007, S. 170). Strukturmodelle sind statische Kausalmodelle und gehen von linearen Gesundheitsverhaltensänderungen aus, die durch die Erhöhung von bestimmten Determinanten entstehen. Eine andere Gruppe von Erklärungstheorien sind die sogenannten stufen- oder Stadienmodelle. Diese Modelle gehen nicht davon aus, dass Verhaltenänderung linear wie in einer mathematischen Gleichung verläuft. Stufenmodelle basieren auf der Grundlage, dass Menschen sich im Bezug auf eine Verhaltensänderung in unterschiedlichen Stadien und unterschiedlichen Zuständen befinden (Lippke & Kalusche, 2007, S. 170). Weiterhin nehmen diese Modelltheorien an, dass nicht alle Personen gleich stark auf bestimmte Einflussgrößen reagieren, da sie sich hinsichtlich ihrer Persönlichkeit unterscheiden. Jedoch setzt dieses Modelldenken die Annahme voraus, dass sich Personen zu bestimmten Stadien zuordnen lassen und die Schwierigkeiten und Probleme der Personen innerhalb dieser Stadien sehr ähnlich und vergleichbar sind. Neben dem Modellwissen um die Determinanten der Sportaktivität muss insbesondere der Anwender von den Interventionen wissen, mit welchen ganz konkreten Maßnahmen, Methoden und Mitteln er auf die relevanten Faktoren der Sportaktivität Einfluss nehmen kann. Dieses Wissen liefern uns nach Fuchs (2003, S.147) die Interventionstheorien. Das Kernstück einer jeden Interventionstheorie ist die ganz konkrete Maßnahme, mit der versucht wird Einfluss auf das Verhalten der Person zu nehmen (Fuchs, 2003, S. 147). Eine Interventionstheorie beschreibt nicht nur eine Maßnahme, sondern berücksichtigt idealerweise wichtige Komponenten wie Effekt, Setting und Zielgruppe (Fuchs, 2003, S. 148). Der Effekt einer Maßnahme beschreibt spezifisch, wie genau eine Maßnahme auf welche entsprechende Größe des Verhaltens wirkt. So kann zum Beispiel durch eine Maßnahme wie die konkrete
Formulierung und schriftliche Fixierung eigener, kurzfristig erreichbarer, jedoch angemessener Bewegungsziele bei Rehabilitationspatienten insbesondere für den Zeitraum nach einer Rehabilitation dazu führen, dass der Faktor Selbstwirksamkeit und Handlungstiefe bei dem Rehabilitanden erhöht wird. Bezüglich des Settings und der Zielgruppe ist darauf zu achten, dass die Maßnahmen der Intervention die entsprechenden sozialen und strukturellen Randbedingungen der Einrichtung und die Bedürfnisse der Zielgruppe berücksichtigt. Eine Intervention im schulischen Kontext mit Kindern und Jugendlichen erfordert sicher eine andere Konzeption als eine Intervention im Bereich der medizinischen Rehabilitation, um entsprechende Effekte zu erzielen. Die Tabelle vier auf der folgenden Seite zeigt die wichtigsten Vertreter der Erklärungstheorien und die dazu passenden Struktur- und Stadienmodelle sowie bekannte Interventionstheorien im Überblick. Bei der genauen Betrachtung erkennt man entsprechende Dopplungen. Bestimmte Theorien wie zum Beispiel die Sozial-kognitive Theorie von Bandura (2000) oder das Transtheoretische Modell von Prochaska und DiClemente (1992) sind sowohl eine Erklärungstheorie als auch eine Interventionstheorie, da diese Theorien erklären, unter welchen Bedingungen eine Verhaltensänderung erfolgen kann und als Interventionstheorie Techniken und Maßnahmen liefern, mit denen die entsprechenden Einflussgrößen des Verhaltens beeinflusst werden können.

Eine detaillierte Beschreibung aller in der Tabeile aufgezählten Modelle würde den Rahmen dieser Arbeit unnötig erweitern und ist auch zum theoretischen Gesamtverständnis nicht nötig. Nachfolgend werden zwei wichtige Vertreter der in der Tabelle vier dargestellten Theorien ausführlicher beschrieben. Die Sozial-kognitive Theorie von Bandura (2000) und das Transtheoretische Modell von Prochaska und Diclemente (1992) sind Modelle mit Doppelcharakter in Bezug auf Erklärungs- und Interventionstheorie und die in der Literatur am häufigste genannten Theorien. Deshalb werden sie an dieser stelle näher erläutert.
Die Sozial-kognitive Theorie von Bandura
Wie bereits erläutert, handelt es sich hierbei erklärungstheoretisch um ein Strukturmodell (vgl. Tab. 4, S.34). Strukturmodelle sind statische Kausalmodelle und gehen von linearen Gesundheitsverhaltensänderungen aus, die durch die Erhöhung von bestimmten
Determinanten entstehen. Die begünstigten Faktoren zur Veränderung des Verhaltens in dieser Theorie sind:
1. die wahrgenommenen begünstigten Faktoren oder Hindernisse
2. die angestrebten Ziele
3. die Handlungsergebniserwartungen
4. die Selbstwirksamkeitserwartung
Bandura definiert die Selbstwirksamkeitserwartung als eine subjektive Einschätzung der eigenen Kompetenzen und Fähigkeiten, eine bestimmte Aufgabe oder ein bestimmtes Problem lösen zu können (Bandura, 1997). Selbstwirksamkeitserwartungen sind somit immer Ausdruck der inneren Überzeugung einer Person. Sätze wie „ Ja das traue ich mir zu“, „ Ja das schaffe ich sicher“ veranschaulichen ein hohe Selbstwirksamkeitserwartung. Diese Prädiktorvariable spielt in der Theorie von Bandura eine bedeutende Rolle, da dieser Faktor auf alle anderen Faktoren einen weiteren Einfluss hat. Selbstwirksamkeitserwartung ist in diesem Modell eine Art Schlüsselfaktor, der sich wie bei einer Dominoreihe auf die anderen Faktoren und auf das Verhalten direkt auswirkt. Ein KHK-Patient, der sich zutraut, dreimal die Woche für 30 Minuten zu joggen zu, wird nach Banduras Theorie auch eher davon überzeugt sein, dass sich das Joggen sich positiv auf die Gesundheit und seine Erkrankung auswirkt. Somit fördert die Selbstwirksamkeit direkt die Handlungsergebniserwartung einer Person. Diese Form der Erwartungen hat immer eine Wenn-Dann-Struktur. Wenn ich etwas Bestimmtes mache, dann rechne ich mit einem bestimmten Ergebnis oder einer Konsequenz (Scholz et al., 2007, S. 138). Eine Person mit einer hohen Selbstwirksamkeitserwartung wird den Hindernissen und Barrieren weniger Bedeutung zukommen lassen, da sie ja davon überzeugt ist, das Verhalten in Zukunft auch durchführen zu können. Ein KHK-Patient der davon überzeugt, ist dass er nicht joggen kann, da er auf Grund des erlittenen Herzinfarktes in seiner kardiovaskulären Leistungsfähigkeit erheblich eingeschränkt ist, wird dies auch nicht planen und nicht als sein Ziel für die nächsten Wochen bestimmen. Diese Person ist jedoch vielleicht davon überzeugt, in Zukunft an stelle des Joggens eine andere Bewegungsform zum Beispiel Nordic Walking durchführen zu können und sieht darin eine gute Alternative zum Joggen. Diese innere Überzeugung steigert die Selbstwirksamkeit und wirkt sich nach Banduras Theorie auf das Zielverhalten der Person aus. Die Person wird mit hoher Wahrscheinlichkeit das Nordic Walking durchführen. Die von Bandura entwickelte Theorie unterscheidet sich von anderen strukturmodellen darin, dass sie nicht nur einen erklärungstheoretischen Ansatz hat, sondern auch interventionstheoretische Techniken und Maßnahmen beschreibt, mit denen man auf die jeweiligen Faktoren Einfluss nehmen kann (Scholz et al., 2007).
